Riedel's thyroiditis association with IgG4-related disease

Abstract: Riedel's thyroiditis is a component of IgG4-RD with the density of the IgG4 +  lymphocytic infiltrate being time dependent. In this small study, we did not identify differences in IgG4 infiltration between RT and HT, minimizing the utility of this marker in RT diagnosis.

“…Fatourechi et al confirmed the presence of IgG4+ plasmacytes in two patients with RT [62]. Stan, et al more recently reported six cases of RT, and three patients showed IgG4+ plasmacytes >10/HPF [64]. Of those 12 patients including our two cases, only one patient met the comprehensive criteria [45].…”

“…The final proof was provided in 2010, by description of a case in which immunohistochemistry for IgG4 showed an intense positive staining (29). The study by Stan et al, although limited to six cases of RT, currently represents the largest published series (32). The relevance of this study stems from the finding that IgG4 staining was most intense in areas of marked inflammation and was virtually absent in fibrotic areas.…”

“…Clinically, the disease commonly manifests as a mass lesion with or without systemic symptoms [21]. Many distinct inflammatory disorders, for example Riedel thyroiditis, have been revealed to be closely connected to IgG4-RD [21,22]. Although its involvement in the parathyroid gland has not been reported and supportive clinical information, for example serum IgG4 level, was not evaluated in this patient before the operation, the characteristic histopathological features observed in this patient indicate that the tumor of this patient has the features of IgG4-RD [20].…”

“…IgG4-RD usually decreases the functions of the involved organs by inducing scar-like fibrosis, e.g. Riedel thyroiditis or autoimmune pancreatitis [22]. However, organ-stimulatory effect of IgG4 was described in a portion of Graves' disease [24,25].…”
