Rising Methicillin-ResistantStaphylococcus aureusInfections in Ear, Nose, and Throat Diseases

Abstract: The increasing incidence of methicillin-resistant Staphylococcus aureus infections (MRSA) in ENT diseases is becoming a big clinical concern. Here two patients are described who developed MRSA infections presented with unusual post-FESS epistaxis and postmastoidectomy perichondrial abscess and failed treatment with broad spectrum intravenous antibiotics. Following treatment with oral linezolid combined with local mupirocin dressing both patients fully recovered.

“…It is well known that nasal colonization with S. aureus along with MRSA, particularly in biofilm form, is associated with CRS disease recalcitrance and poor outcomes after sinus surgery [ 27 , 28 , 29 , 30 ]. Biofilms are thought to be the main mediators for disease persistence and treatment failure in various chronic disorders including CRS [ 30 ].…”

“…Rapid onset, severe swelling, tenderness and erythema have been observed before in MRSA head and neck infections, and, when encountered, should raise the index of suspicion of MRSA otitis for the clinician. 22 Patients with MRSA tended to experience a longer time to resolution of symptoms. MRSA AOE tended to be associated with diabetes, immunocompromise and comorbidities to a greater degree than MSSA.…”

“…While little is known about MRSA in AOE, increasing rates of MRSA in soft tissue infections 21 and in otolaryngology, 16 , 22 are well-documented. The increased incidence observed in our study, when compared to the earlier findings of Roland et al 3 appear to be consistent with the global trend of increasing prevalence of community-acquired MRSA carriage 18 and infection 17 , 22 over the past 20 years.…”
