Risk Factors for Stroke in Subjects With Normal Blood Pressure

Abstract: Background and Purpose-Although stroke is strongly associated with hypertension, some individuals with normal blood pressure (BP) experience a stroke. This prospective study explored risk factors for stroke in subjects with normal BP. Methods-A total of 11 228 men and 17 174 women, 45 to 73 years old, were examined in a population-based cohort study. Normal BP was defined as BP Ͻ140/90 mm Hg and no treatment for hypertension. The incidence of stroke was followed over a mean period of 6 years. Results-In the co… Show more

“…36 Patients with hypertension are exposed to atherosclerotic risk and target organ damage for long periods both before and even during therapy. 4 Thus, besides the greater severity of hypertension, treatment implies other cardiovascular risk factors, including undetectable residual confounding factors in addition to the classical risk factors, 37 such as chronic kidney dysfunction or blood vessel endothelial lesion, which leads to a greater risk compared with untreated individuals. The review of intervention trials by Zanchetti 38 also indicates that preexisting high risk sets a ceiling effect to the benefits of treatment and underlines the limitations of late interventions.…”

“…9 -11 In a recent large, population-based study from Malmö, Sweden among normotensive men and women, BMI was a statistically significant and independent predictor for stroke. 40 Furthermore, the risks of excessive body fat for stroke can be obscured by a history of cigarette smoking and thinness associated with deteriorating health. 41 In addition to the established mechanisms by which BMI affects stroke risk such as hypertension and diabetes, other mechanisms may be associated with the increased risk.…”

“…Recently, several other possible factors have been suggested to play a role⎯i.e., hyperhomocysteinemia, inflammation, infection, and hypercoagulability (23,24)⎯although further evidence will be needed to confirm these relations. Nevertheless there are still many deteriorating factors to be remained undetermined (16,17), so we have to break such enigmas and promote the strategies for preventing and treating cerebrovascular disease (25).…”

“…But there are many cases of such cerebrovascular disease of undetermined cause (16,17). So, understanding more about these is very important for preventing and treating cerebrovascular disease.…”
