Robot‐assisted radical prostatectomy in the Korean population: A 5‐year propensity‐score matched comparative analysis versus open radical prostatectomy

Abstract: Robot-assisted laparoscopic radical prostatectomy represents an effective surgical approach for the treatment of prostate cancer in the Korean population, as it provides equivalent oncological outcomes to open radical prostatectomy.

“…All of the included studies reported exclusion/inclusion criteria [ 7 39 ]. The 19 studies [ 8 , 10 , 14 , 16 20 , 22 , 23 , 27 , 29 , 30 33 , 36 38 ] that compared the outcomes between ORP and RARP groups included 16 830 prostate cancer patients. Eleven of these studies [ 7 , 9 13 , 15 , 21 , 34 , 35 ] compared the outcomes between LRP and RARP, and 5 studies [ 10 , 24 26 , 39 ] compared the outcomes between ORP and LRP.…”

“…Our results thus differed from the previous studies. The prostatic apex was reported to be the most common location of PSM, and improved visualization of the prostatic apex during RARP may reduce the risk of PSM [ 8 , 50 53 ].…”

“…However, the results showed that PSM rates in pT2 cancers was lower in RARP (OR:0.77; 95% CI: 0.63 to 0.95; P = 0.01)(Figure 9 ). Four studies [ 20 , 43 , 60 , 73 ] comparing mean lymph node yield and the results showed that lymph node yield is higher in RARP (WMD: 1.61; 95% CI: 1.18 to 2.05; P < 0.001)( Figure S9 ), and 16 studies [ 20 , 26 , 33 , 34 , 39 , 49 , 58 , 61 , 64 - 68 , 73 , 84 , 85 ] reported on positive lymph node, There was a statistical differences decreased positive lymph node in RARP than RRP (OR:0.45; 95% CI: 0.31 to 0.65; P < 0.001)(Figure 10 ).…”

“…In our previous report, we found that RARP and ORP had an equal oncological outcome without a significant difference in terms of 5-year BCRFS (P = 0.651), MFS (P = 0.876) and CSS (P = 0.076). 29 Furthermore, in a recent systematic review and meta-analysis comparing the oncological outcomes of RARP and ORP, the authors showed similar overall PSM rates (RARP vs ORP: OR 1.21; P = 0.19) and BCRFS rates (RARP vs ORP: HR 0.9; P = 0.526). 30 Regarding laparoscopic radical prostatectomy, Guillonneau et al reported 91% BCRFS at 3-year follow up in 1000 patients; in addition, Touijer et al showed overall 5-and 8-year BCRFS rates of 78% and 71%, respectively.…”
