Routine short course pre-op radiotherapy or selective post-op chemoradiotherapy for resectable rectal cancer? Preliminary results of the MRC CR07 randomised trial

Abstract: 3511 Background: Radiotherapy reduces local recurrence (LR) when combined with surgery in resectable rectal cancer. The Medical Research Council CR07 trial was designed to compare two different radiotherapy policies when combined with mesorectal excision. Methods: Patients with operable non-metastatic adenocarcinoma of the rectum were randomised to either routine pre-op short course radiotherapy [25Gy/5F] (PRE) or selective post-op chemoradiotherapy (POST) for patients with involvement of the circumferential … Show more

“…The group demonstrated a significant reduction in local recurrence with preoperative radiotherapy compared with surgery alone, although, surprisingly, no improvement in survival. Emerging results from the UK CR07 trial appear to confirm a reduction in local recurrence and, in contrast to the Dutch results, an improvement in survival16. However, these results are only preliminary.…”

“…Most recent publications arbitrarily define the rectum as composed of three parts, namely the low, mid and upper rectum, with the height of the cancer defined as the lower edge of the tumour5, 41. In many instances this has been reported as up to 5 cm being low rectum, 5–10 cm mid rectum and 10–15 cm upper rectum5, 16. However, these definitions are ambiguous as a 5‐cm tumour could be low or mid rectal.…”

“…A short course (typically 1 week) of preoperative pelvic radiotherapy further reduces the rate of local recurrence of operable rectal cancer [6,8]. However, this has no benefit if the CRM is contaminated with tumour (within a millimetre of the CRM) [2,6].…”

“…Patients with positive nodes received adjuvant chemotherapy. Local recurrence was reduced (4.7% vs 11%) and 3-years survival was better (79.5% vs 74%) in the group of preoperative RT [27].…”
