Blockchain technology is found to have its applicability in almost every domain because of its advantages such as crypto-security, transparency, immutability, decentralized data network. In present times, a smart healthcare system with a blockchain data network and healthcare 4.0 processes provides transparency, easy and faster accessibility, security, efficiency, etc. Healthcare 4.0 trends include industry 4.0 processes such as the internet of things (IoT), industrial IoT (IIoT), cognitive computing, artificial intelligence, cloud computing, fog computing, edge computing, etc. The goal of this work is to design a smart healthcare system and it is found to be possible through integration and interoperability of Blockchain 3.0 and Healthcare 4.0 in consideration with healthcare ground-realities. Here, healthcare 4.0 processes used for data accessibility are targeted to be validated through statistical simulation-optimization methods and algorithms. The blockchain is implemented in the Ethereum network, and with associated programming languages, tools, and techniques such as solidity, web3.js, Athena, etc. Further, this work prepares a comparative and comprehensive survey of state-of-the-art blockchain-based smart healthcare systems. The comprehensive survey includes methodology, applications, requirements, outcomes, future directions, etc. A list of groups, organizations, and enterprises are prepared that are working in electronic health records (EHR), electronic medical records (EMR) or electronic personal records (EPR) mainly, and a comparative analysis is drawn concerning adopting the blockchain technology in their processes. This work has explored optimization algorithms applicable to Healthcare 4.0 trends and improves the performance of blockchain-based decentralized applications for the smart healthcare system. Further, smart contracts and their designs are prepared for the proposed system to expedite the trust-building and payment systems. This work has considered simulation and implementation to validate the proposed approach. Simulation results show that the Gas value required (indicating block size and expenditure) lies within current Etherum network Gas limits. The proposed system is active because block utilization lies above 80%. Automated smart contract execution is below 20 seconds. A good number (average 3 per simulation time) is generated in the network that indicates a health competition. Although there is error observed in simulation and implementation that lies between 0.55% and 4.24%, these errors are not affecting overall system performance because simulated and actual (taken in state-of-the-art) data variations are negligible.

Technological innovations have enabled the realization of a utopian world where all objects of everyday life, as well as humans, are interconnected to form an “Internet of Things (IoT).” These connected technologies and IoT solutions have led to the emergence of smart cities where all components are converted into a connected smart ecosystem. IoT has envisioned several areas of smart cities including the modern healthcare environment like real-time monitoring, patient information management, ambient-assisted living, ambient-intelligence, anomaly detection, and accelerated sensing. IoT has also brought a breakthrough in the medical domain by integrating stake holders, medical components, and hospitals to bring about holistic healthcare management. The healthcare domain is already witnessing promising IoT-based solutions ranging from embedded mobile applications to wearable devices and implantable gadgets. However, with all these exemplary benefits, there is a need to ensure the safety and privacy of the patient’s personal and medical data communicated to and from the connected devices and systems. For a smart city, it is pertinent to have an accessible, effective, and secure healthcare system for its inhabitants. This paper discusses the various elements of technology-enabled healthcare and presents a privacy-preserved and secure “Smart Medical System (SMS)” framework for the smart city ecosystem. For providing real-time analysis and responses, this paper proposes to use the concept of secured Mobile Edge Computing (MEC) for performing critical time-bound computations on the edge itself. In order to protect the medical and personal data of the patients and to make the data tamper-proof, the concept of blockchain has been used. Finally, this paper highlights the ways to capture and store the medical big data generated from IoT devices and sensors.

Smart Contract-Based Agricultural Food Supply Chain Traceability

The complexity of a supply chain makes product safety or quality issues extremely difficult to track, especially for the basic agricultural food supply chains of people's daily diets. The existing agricultural food supply chains present several major problems, such as numerous participants, inconvenient communication caused by long supply chain cycles, data distrust between participants and the centralized system. The emergence of blockchain technology effectively solves the pain-point problem existing in the traceability system of agricultural food supply chains. This paper proposes a framework based on the consortium and smart contracts to track and trace the workflow of agricultural food supply chains, implement traceability and shareability of supply chains, and break down the information islands between enterprises as much as possible to eliminate the need for the central institutions and agencies and improve the integrity of the transaction records, reliability and security. At the same time, farmers record details of the environment and crop growth data in the InterPlanetary File System (IPFS) and store file IPFS hashes in smart contracts, which not only increases data security but also alleviates the blockchain storage explosion problem. This framework has been applied in Shanwei Lvfengyuan Modern Agricultural Development Co., Ltd. Although there are still many defects, the framework has successfully realized functions such as disintermediation and tracing of agricultural product information through QR codes.
