Sacral insufficiency fractures: current concepts of management

Abstract: Insufficiency fractures represent a special category of stress fractures that occur in bones with reduced mineral content and elastic resistance. SIFs, a well-defined subgroup of the latter group, are not uncommon, but lack of clinical suspicion results in many being undiagnosed. SIFs are set to become an important clinical entity of both social and economic significance as the Western population ages. Subtle clinical presentations and signs coupled with radiographic findings that can mimic other unrelated or … Show more

“…However, although plain X-rays are considered an adequate tool for diagnosis of anterior ring pathologies, their accuracy in the evaluation of posterior pelvic ring lesions is limited, due to the low bone density, the interposing viscera [6,16], and the minimal initial displacement of FPFs. As a result sacral insufficiency fractures are often overlooked in plain X-rays of patients presenting with low back and/or pelvic pain [20]. Fractures of the pubis, which are visible on the X-ray, are nearly always associated with injuries of the posterior pelvic elements [21].…”

“…1 SIFs are often bilateral and commonly associated with other fractures, most frequently a pubic ramus fracture. [2][3][4] However, it remains unclear which fracture (sacral or pubic rami) occurs first, and in cases of bilateral SIFs, whether fractures occur simultaneously or sequentially.…”
