SAT0260 Ovarian function preservation with gonadotropin-releasing hormone analogues in patients with systemic lupus erythematosus treated with cyclophosphamide

Abstract: BackgroundThe fertility in childbearing Systemic Lupus Erythematosus (SLE) patients can be impaired due to several conditions. In particular, treatment with alkylating agents, as cyclophosphamide (CYC), could determine menstrual irregularities and premature ovarian failure (POF). Gonadotropin-releasing hormone analogues (GnRH-a) is one of the preventive strategy suggested by the recently published EULAR recommendations (1). They are characterized by good safety profile and effectiveness in reducing POF rate in… Show more

“…In addition POF was further defined in three studies as amenorrhea for at least 12 months and FSH levels ≥ 40 mIU/ mL 6,15,26 ; in one study each by (i) sustained amenorrhea, estradiol concentrations < 100pmol/L, and FSH > 25 IU/L 4 ; (ii) FSH ≥ 23 mIU/mL, LH ≥ 15.9 mIU/mL, and estradiol < 37 pg/mL 26 ; (iii) low estradiol levels, high FSH levels, and menstrual irregularity 27 ; and (iv) measuring FSH and estradiol levels but cutoff values were not provided. 28 Among these 218 women, a total of 132 (60.5%) received GnRHa concurrently with IV CYC. Ovarian preservation was successfully achieved in 125 out of 132 (94.6%) patients receiving concurrent GnRHa with CYC compared to 50 out of 86 (58.1%) in the group that did not receive concurrent GnRHa (Table 2).…”
