Scleral buckling versus primary vitrectomy in rhegmatogenous retinal detachment study (SPR study): Risk assessment of anatomical outcome. SPR study report no. 7

Abstract: Primary anatomical success is an important result for patients undergoing RRD surgery. Our data demonstrate that the final anatomical outcome is related to a higher preoperative number of breaks and cryotherapy in phakic eyes. Additional risk factors varied between phakic and pseudophakic subgroups. Our findings may be used to facilitate the prognosis of future patients with RRD.

“…Indeed, subsequent analysis of the SPR data suggested that the presence of a posterior YAG capsulotomy (suggesting a history of, and likely residual media opacity) predicted poorer outcomes for scleral buckling. [7] Endoilluminator-assisted scleral buckling may improve on this limitation.…”

“…• Eye with native lens (phakic), no previous surgery (14-17, 31-33, e54) • Hole clearly discernible, not too large (12,34) • No or only slight PVR reaction (12,16,34) • Good view of site. This is the case in almost half of all patients with retinal detachment (5).…”

“…There have been several clinical trials comparing the two methods 1116. The scleral buckling vs primary vitrectomy in RRD (SPR) study16 was the largest randomized clinical trial, and it showed that anatomic and functional outcomes of the two methods were comparable.…”
