Secondary thyroidectomy: A twenty‐year experience

Abstract: Secondary thyroidectomy is an operation generally considered to be associated with a significantly increased risk of damage to the recurrent laryngeal nerves and parathyroid glands. During a 20‐year period, to December, 1986, a total of 408 secondary thyroidectomies were performed. The majority (n=227) were for recurrent nodular goiter, followed by reoperations for thyroid cancer (n=151), and operations for secondary thyrotoxicosis (n=30). The incidence of operative recurrent laryngeal palsy was 1.5% over the … Show more

“…Patients with malignancy in DO group needed re-surgery for completion thyroidectomy, but no re-surgery performed on cases of TT group. Due to the small remnant tissue, re-operation is difficult and carries a significant risk of damage to both RLNs and the parathyroid glands, and during completion thyroidectomy, there is a 10-fold increase in iatrogenic injuries [17]. So, due to the high incidence of malignancy in this survey (10 %), TT can be the method of choice for MNG surgery.…”

“…In our study, the rate of permanent hypoparathyroidism was also 0%. The low rates of permanent hypocalcemia after reoperation may be due to the use of operative techniques that aim to preserve the vascular pedicle of the parathyroid glands after capsular dissection (1,19).…”

“…This fact may not be recognized unless long-term data are reviewed after primary nontotal tyroidectomy. According to Reeve and others the peak incidence of the second operation appears 13 years after subtotal thyroidectomy (3,30).During a subtotal thyroidectomy it is also possibility to remain the small posterior-lateral part of the thyroid tissue (Zuckerkandl`s tuberculum) (13,20,31).…”
