Sedation for bronchoscopy in children: A prospective randomized double‐blinded trial

Abstract: Introduction In pediatric patients, flexible bronchoscopy requires deep sedation. Different sedation regimes are common, but only some of them include opioids. Due to their antitussive effect, the use of short‐acting opioids may be beneficial for this particular indication, but additional respiratory depression may lead to an increase in adverse events. Here, we systematically compared sedation regimes in children undergoing flexible bronchoscopy with either propofol alone, or a combination of propofol and rem… Show more

“…The results suggest that a relatively high dose of remifentanil was used in the cough suppression group, and there was a significant difference in extubation response. This suggests that remifentanil can effectively inhibit the cough response, which is consistent with the results of a previous study ( Tschiedel et al, 2021 ). Although the times for extubation and the time for awakening were slightly longer in the cough suppression group than in the cough group, the differences between the two groups were not statistically significant.…”

“…The control of coughing by opioids may be outweighing the risk for hypoventilation and apnea ( Tschiedel et al, 2021 ). Because of the disadvantages of fentanyl in terms of offset time, hepatic metabolism, and accumulation and prolonged effect with continuous infusion, we chose RF as the preferred opioid in this study ( Lin et al, 2019 ).…”

“…Propofol se daje za sedaciju kao indukcioni bolus 1 mg/kg tokom 30 sekundi, a onda kao dodatni bolus 1-2 mg/kg na 3-5 minuta ili kontinuirano 6-10 mg/kg/h. Neželjeni efekti su iritacija vene, bradikardija, hipotenzija, respiratorna depresija, pruritus, a u slučaju intoksikacije, nastaju aritmije, srčani zastoj, depresija CNS-a i apneja 12,18,19 .
