Selection tool alpha-fetoprotein for patients waiting for liver transplantation: How to easily manage a fractal algorithm

Abstract: Alpha-fetoprotein (AFP) behavior in patients with hepatocellular carcinoma (HCC) waiting for liver transplant (LT) represents a perfect biological example of a fractal model in which its progressive modification and possible future prediction of its values are very hard to capture. As a consequence, AFP represents a useful but poorly manageable tool to increase the ability to better select HCC patients waiting for LT. Trying to find a "filrouge" in the recent literature, no definitive answers can be done to se… Show more

“…Transplant oncology is not a new concept; however, it has recently attracted renewed attention. 1 , 2 The term transplant oncology was first introduced in 2015 3 and the first international consensus meeting on transplant oncology was held in 2019. This meeting resulted in the publication of the first international consensus guidelines on the topic.…”

“…Several authors have proposed selection models with integrated AFP [13,14,23,30,31], sometimes combined with other biological markers [9,22]. Single AFP cut-off levels have been used, either for inclusion of additional patients outside criteria with low AFP levels [10,30,32] or for exclusion of patients within criteria, but with high AFP levels [23,33].…”

“…AFP is a cytosolic protein secreted by liver precursor cells. As the cells gradually mature, they no longer express AFP (Lai et al, 2015;Takeyama et al, 2015). ALB, synthesized by liver cells and secreted into the circulatory system, is the most abundant protein in the plasma of healthy human beings.…”
