Self-poisoning with oral salbutamol.

Abstract: ratings rose as the drug was withdrawn but subsequently subsided, suggesting that the symptoms represented a true withdrawal syndrome and not the revival of the original anxiety. Furthermore, the perceptual changes, such as intolerance to light and sound, unsteadiness, and a feeling of motion, are untypical of anxiety. Some patients have complained of strange smells and a metallic taste. Both our patients were originally taking other benzodiazepines before transferring to clobazam. They developed a typical syn… Show more

“…[12][13][14] Wiley et al 10 reported that the threshold dose for the development of 3 or more signs of toxicity is 1 mg/kg or 3 to 10 times the normal daily dose. Our patient ingested 1.9 mg/kg and presented with agitation, tremor, sinus tachycardia, mild hypokalemia, and hyperglycemia.…”

“…This case emphasises the importance of a high index of suspi cion for rhabdomyolysis in drug overdosage. It also sug gests that ß2-adrenoceptor agonist overdosage may be more dangerous than is sometimes suggested [7],…”

“…[49,62,66] Minor metabolic effects of salbutamol include slight increases in blood glucose, insulin, free fatty acids, lactate, pyruvate and ketones, and hypokalaemia, hypomagnesaemia, hypocalcaemia and hypophosphataemia. [49,66,72,76,77] Some nebulised salbutamol solutions contain an additive, benzalkonium chloride, which is associated with a 3% incidence of induced bronchoconstriction in patients with asthma (Hendeles L, personal communication).…”

“…The margin of safety is wide, and nonfatal oral overdoses of 20 times the usual dose have been reported. [72] Salbutamol has effects on other adrenergic systems, including a dose-related increase in heart rate. This effect is more pronounced after intravenous than after aerosol administration, occurs within 45 minutes of a dose and subsequently remains stable or declines.…”
