Background The incidence of ulcer pressure in the high care unit (HCU) was relatively high and could be reliably predicted using tools such as the Norton and Jackson/Cubbin scales. However, other risk factors, such as age, gender, consciousness, systemic condition, duration of treatment, and use of restraint, may contribute to the occurrence of ulcer pressure. This study was conducted to analyze the relationship of various risk factors for pressure ulcers and prediction of ulcer pressure, using Norton and Jackson/Cubbin scale, to incident pressure ulcers in HCU patient. Methods This study utilized a prospective cross-sectional study design to analyze various risk factors for ulcer pressure development in a patient admitted to the HCU, including age, gender, blood profile, consciousness, duration of treatment, and use of restraint. The Norton and Jackson/Cubbin scale was employed to predict pressure ulcers. The relationship between the risk factors and the prediction of pressure ulcer incidents was evaluated using multiple logistic binary regression analysis. Result Both the Norton and Jackson/Cubbin scales predicted a lower risk of pressure ulcer development (60.98 and 99.02%, respectively). This prediction is consistent with the low incidence of pressure injuries found, which is only 4.39%. Furthermore, the relationship between the identified risk factor (gender, duration of treatment in HCU and use of restraint) and the prediction and incident of pressure ulcer was not significant (p > 0.05). Thus, it is suggested that these risk factors may not strong predictors of pressure ulcer development. Conclusion This study's result indicated no significant relationship exists between possible identified risk factors and the development of pressure ulcers in HCU patients. However, the Norton and Jackson/Cubbin scales were reliable predictors of pressure ulcer occurrence, with both scales predicting a lower risk of pressure ulcer development.

Objetivo: avaliar os fatores associados à ocorrência de lesão por pressão em pacientes críticos, adultos em unidade de terapia intensiva. Método: estudo transversal realizado na unidade de terapia intensiva de um hospital de ensino no estado de Sergipe, entre agosto/2018 e julho/2019. Foram incluídos pacientes internados com idade maior ou igual a 18 anos e tempo de permanência mínima de 24 horas. Aos pacientes elegíveis e conscientes foram feitos convites para participar de forma voluntária e oferecidos esclarecimentos quanto ao consentimento. No caso dos pacientes inconscientes a autorização foi solicitada ao familiar ou representante legal. E para ambos os casos foi aplicado o Termo de Consentimento Livre e Esclarecido. Para a análise dos fatores de risco associados à lesão por pressão utilizaram-se os testes exato de Fisher, qui-quadrado, Shapiro-Wilks, Mann-Whitney, regressão linear e logística, com significância de 5%. Resultados: a casuística foi composta de 99 pacientes. Destes, 30 (30,3%) desenvolveram lesão por pressão. A maioria era do sexo feminino, com idade média 65±14 anos e comorbidades prévias como diabetes, infarto agudo do miocárdio e acidente vascular encefálico. Quando avaliados os fatores de risco para o desenvolvimento de lesão por pressão, observou-se que a lesão renal aguda aumentou 3,5 vezes essa chance (p=0,036) e para cada dia a mais de internação o paciente apresentou 3,5 vezes mais chances de desenvolver uma nova lesão por pressão (p=0,038). Conclusão: a lesão renal aguda e o tempo de internação na unidade intensiva foram fatores de risco associados ao desenvolvimento de lesão por pressão.

Background: The intensive care unit has a much higher pressure ulcers incidence than general wards. Predicting and eliminating the risk of pressure ulcer development is thus of utmost importance in health care, including clinical nursing practice. Aim:The meta-analysis aimed to examine the predictive validity of the Jackson-Cubbin Scale.Methods: This study is a systematic literature review and meta-analysis. Research articles published in periodicals indexed targeted the critical care patients and offered sufficient data(true-positive, false-positive, true-negative and false-negative results) in the EBSCO, PubMed, Ovid, Web of Science and Cochrane databases, as well as those from two journals indexed in a Chinese database (Wangfang Data and China National Knowledge Infrastructure) were reviewed. Preferred reporting items for systematic reviews and meta-analyses were used; the internal validity of the diagnostic studies was analysed using Quality Assessment of Diagnostic Accuracy Studies 2. Sensitivity, specificity, positive and negative likelihood ratios were counted by STATA 15.0.Results: Nine diagnostic studies with high methodological quality, involving a total of 7684 patients, were retrieved and assessed. This meta-analysis showed that the pooled sensitivity, specificity and area under the symmetric summary receiver operating characteristic curve of the Jackson-Cubbin Scale were 0.81 (95% confidence interval [CI], 0.58-0.88), 0.76 (95% CI, 0.51-0.95) and 0.84 (95% CI, 0.81-0.87), respectively. Conclusions:The study suggests that the Jackson-Cubbin Scale has a moderate level of predictive validity. Moreover, Jackson-Cubbin's predictive validity could be increased if it was implemented differently based on the attributes of the research participants.Relevance to Clinical Practice: The Jackson-Cubbin Scale has a moderate level of predictive validity with good sensitivity and specificity for the determination of the risk of PUs in adult ICU patients. Bedside nurses could use the Jackson-Cubbin scale for dailly assesment for ICU patients.
