Comparison of Patients' Admission, Mean and Highest Sofa Scores in Prediction of ICU Mortality: a Prospective Observational Study

Background:Use of valid criteria for evaluation of patients admitted to intensive care unit (ICU) is essential to assess the quality of services provided. This study was conducted to evaluate admission, mean and the highest Sequential Organ Failure Assessment (SOFA) scores in predicting mortality and outcome of patients admitted to the ICU.Methods:This prospective observational study was conducted on 300 patients admitted to the ICU of Imam Khomeini Hospital, Sari, Iran, in 2015. The SOFA tool was applied at the beginning of the admission of patients in the ICU and then every 24 hours. Functions of six vital organs were recorded on a daily basis for scoring with SOFA. Demographic profile, the main diagnosis leading to hospitalization in the ICU, previous interventions, complications and hospital outcomes were recorded for each patient. The patients’ admission, mean and highest SOFA scores were also evaluated and recorded during the study period. These scores were compared between deceased and survived patients. ROC curve determined the best cut-off point of score calculated.Results:Among all 300 patients who were enrolled in the study, 189 (63%) cases were males. Trauma was the most common cause of hospitalization and mortality. Thirty point seven percent of 92 deceased patients needed to intubation. Mean length of ICU and hospital stay were significantly longer for deceased patients than the survivors (P <0.001). The admission, mean and highest SOFA scores were 11.72, 16.38 and 16.45 in deceased patients, as well as 6.52, 5.82 and 6.5 in survived patients, respectively. The area under the curve (AUC) for the admission, mean and highest SOFA scores were 0.875, 0.988 and 0982, respectively. All three models were able to predict the outcome of patients significantly (P <0.0001). The cut-off point of 10.6 for the mean SOFA had the highest sensitivity and specificity in predicting mortality.Conclusion:The results of this study showed that the mean SOFA score had the highest sensitivity and specificity in prediction of ICU mortality. Therefore, this criterion is a valuable indicator to better predictions of mortality and morbidity rate in the ICU patients, which can lead to appropriate health care and therapeutic interventions in these patients.

Objective: To assess the nursing workload using the Nursing Activities Score (NAS) and compare results between patients with and without sepsis at an adult intensive care unit. Methods: Prospective cohort study of patients admitted to the adult intensive care unit from March 10 to December 31, 2008. Clinical data of septic patients were collected in addition to data needed to calculate scores. Data were collected each day until release from the intensive care unit or up to 90 days of hospitalization. The nursing workload assessed using the Therapeutic Intervention Score System-28 and Nursing Activities Score relative to patients with and without sepsis were compared. The significance level was established at 5%. Results: A total of 437 patients were assessed (318 with and 119 without sepsis). The average nursing workload as measured by the Nursing Activities Score was high for the general intensive care unit (Nursing Activities Score = 74.7%). Patients with sepsis were associated with greater nursing workloads during their first two weeks in the intensive care unit compared with patients without sepsis (p < .05). Patients with septic shock were associated with greater nursing workloads (median Nursing Activities Score = 75.62%, p < .001) compared with patients without sepsis (median Nursing Activities Score = 72.57%), those with sepsis (median Nursing Activities Score = 70.20%), and those with severe sepsis (median Nursing Activities Score = 72.65%). Conclusions: Sepsis increased nursing workload during the first two weeks after admission to the intensive care unit and the NAS may be useful to capture these differences.

Previous studies have shown controversial results of factors associated with short-term mortality in patients with extended-spectrum beta-lactamase (ESBL)-producing E. coli bacteremia and no research has investigated the impact of the geriatric assessment criteria on short-term mortality. Our objective was to determine whether dementia and walking ability are associated with 30-day mortality in patients with ESBL-producing E. coli bacteremia. All blood bottle cultures, analyzed from January 2008 to April 2015, in the Bacteriology Department of a 2,600-bed, university-affiliated center, Nantes, France, were retrospectively extracted. Factors associated with short-term mortality in patients with ESBL-producing E. coli bacteremia: 140 patients with an ESBL-producing E. coli bloodstream infection were included; 22 (15.7%) patients died within 30 days following the first positive blood bottle culture of ESBL-producing E.coli. In multivariate analysis, a reduced ability to walk (OR = 0.30; p = 0.021), presence of dementia (OR = 54.51; p = 0.040), a high Sepsis-related Organ Failure Assessment (SOFA) score (OR = 1.69; p < 0.001), presence of neutropenia (OR = 12.94; p = 0.049), and presence of a urinary tract infection (OR = 0.07; p = 0.036), were associated with 30-day mortality. Our findings provide new data showing an independent association between 30-day mortality with dementia and reduced walking ability, in patients with ESBL-producing E. coli bacteremia. These criteria should be considered in the therapeutic management of patients with ESBL-producing E. coli bacteremia.
