Glucocorticoids are frequently prescribed drugs with important side-effects such as glucose intolerance and tissue remodeling. The goal was to explore the molecular basis of the response of skeletal muscle and adipose tissue during a short-term dexamethasone treatment to better understand the induction of side-effects of glucocorticoids on these metabolic tissues. Fifteen healthy male subjects were assigned to a 4-day treatment with dexamethasone at 4 mg/day. The primary outcome measures were changes in gene expression profiling of subcutaneous skeletal muscle and adipose tissue. Urinary cortisol, plasma, and metabolic biochemistry were also assessed. In both tissues the prominent observation was a response to stress and increased inflammatory responses. An upregulation of the serum amyloid A was detected in skeletal muscle, adipose tissue, and plasma, whereas circulating levels of C reactive protein, another acute phase protein, decreased along with a worsened insulin sensitivity index. As tissue-specific features, tissue remodeling was shown in skeletal muscle while the adipose tissue exhibited a decreased energy metabolism. Several limitations might be raised due to the small number of subjects investigated: a possible cross talk with the mineralocorticoid receptor, and a single time point may not identify regulations occurring during longitudinal treatment. In line with the known physiological effect of glucocorticoids the early modulation of stress response genes was observed. An unexpected feature was the upregulation of the inflammatory and immune pathways. The identification of novel impact on two glucocorticoid target tissues provides a molecular basis for the design of more specific glucocorticoids devoid of adverse effects.

Background Takayasu arteritis is a rare systemic vasculitis, which affects the aorta and its major branches, especially in young females. Diagnosis and treatment for Takayasu arteritis with coronary stenosis are important to prevent fatal complications. Immunosuppressive treatment such as corticosteroid is a common treatment for this condition. However, the effects of immunosuppressive treatment on inflammatory coronary stenosis caused by Takayasu arteritis remains unknown. Case presentation An 18-year-old female had chest oppression on effort and was referred to our hospital due to ST-segment depression in I, aV L , and V 2–4 on electrocardiogram. Coronary angiography showed severe stenosis in the ostium of both the left main trunk and the right coronary artery. 18 F-fluorodeoxyglucose (FDG) positron emission tomography/computed tomography showed isolated inflammation of the aortic root. She was diagnosed with Takayasu arteritis and treated with combined immunosuppressive treatment with corticosteroid and tocilizumab, which decreased the FDG uptake in the aortic root. Four months after initiation of the immunosuppressive treatment, coronary angiography showed regression of the coronary ostial stenosis. Coronary artery bypass surgery was considered, but the patient rejected invasive revascularization for coronary artery disease. She did not have chest oppression or ST-segment depression after the immunosuppressive treatment. She had no cardiac events for 6 months after discharge. Conclusions We described regressed coronary ostial stenosis in a young female patient with Takayasu arteritis. Immunosuppressive treatment might have a favorable effect on coronary ostial stenosis in Takayasu arteritis.

Zusammenfassung Die Riesenzellarteriitis (RZA) ist die h?ufigste systemische Vaskulitis im Erwachsenenalter, und betrifft die gro?en und mittelgro?en Arterien. Die Takayasu Arteriitis (TA) ist auch eine Vaskulitis der gro?en Gef??e und tritt Im Gegensatz zur RZA typischerweise vor dem 40. Lebensjahr auf. Sie ist die dritth?ufigste Vaskulitis im Kindesalter. Beide Gro?gef??vaskulitiden k?nnen zu gef?rchteten isch?mischen Komplikationen im Bereich der Extremit?ten oder der viszeralen Organe f?hren. Aus diesem Grunde ist eine fr?hzeitige Diagnosestellung von gro?er Bedeutung. Die h?ufigste Komplikation der RZA ist die anteriore isch?mische Optikusneuropathie, welche unbehandelt zur Erblindung f?hren kann. In jedem Fall soll die Diagnose histologisch oder sonografisch best?tigt werden. Die Temporalarterienbiopsie oder Bildgebung sollte so fr?h wie m?glich stattfinden, den Therapiebeginn bei dringendem klinischem Verdacht jedoch nicht verz?gern. Desweiteren haben bei der TA die Angiografie und bei beiden Gro?gef???vaskulitiden im Hinblick auf die Erstdiagnostik und die Verlaufsbeobachtung die Magnetresonanzangiografie, die CT-Angiografie, die Positronenemis?sionstomografie und nicht zuletzt der Ultraschall einen hohen Stellenwert. Sowohl die RZA als auch die TA zeichnen sich durch ein Ansprechen auf Kortikosteroide aus. Die Rolle von Immunsuppressiva ist noch nicht vollst?ndig gekl?rt. Bei der Behandlung der RZA liegen derzeit zu Methotrexat als Kortikosteroid sparende Therapie die meisten Daten vor. Im Rahmen der Takayasu Arteriitis haben die TNFalpha-Blocker eine Bedeutung.
