“…5 The bone loss is pronounced in glucocorticoid treated patients' 3 4but is also present in patients treated solely with non-steroidal antirheumatic drugs.' 3 4 It has not been established whether RA-associated osteopenia, if uninfluenced by glucocorticoids, is determined by the degree of functional impairment (disability, impaired ambulation),6-9 or by the duration of the disease.'…”

“…Overt clinical evidence of liver disease in uncomplicated rheumatoid arthritis is considered to be uncommon, although hepatomegaly has been reported in up to 20 % of patients (Webb et al, 1975;Tiger et al, 1976). Abnormalities in the biochemical tests of liver function are more frequent, with elevated serum alkaline phosphatase levels being found in up to 50% of rheumatoid arthritis patients (Lefkovits and Farrow, 1955;Cockel et al, 1971;Webb et al, 1975). Concomitant increases in gammaglutamyl, transpeptidase and leucine amino-peptidase levels suggest that the alkaline phosphatase may have been of hepatic rather than bone origin.…”

“…AP of the bone type and 5 0 nucleotidase are found in the synovial fluid of RA patients to a greater extent than in osteoarthritis [91,92]. AP correlates with an increase in 5 0 nucleotidase [83], and drops with corticosteroids [90]. Gamma glutamyl transaminase (GGT) is raised in 23-77% of patients and correlates with active disease [86,93].…”

“…The spleen can spontaneously rupture even without splenomegaly, perhaps due to involvement of the capsule with the rheumatoid process [82]. Transaminases and bilirubin are generally normal in RA [83,84].…”
