Serum CA 125 and first trimester abortion

Abstract: Our findings are in accordance with the hypothesis of a tropho-decidual origin of this marker, suggesting its possible usefulness in the prognostic evaluation of first trimester threatened abortion.

“…The sensitivity and specificity attained using a single cutoff level of serum progesterone level above 45 nmol/l was 88%, which may represent improvement over other single tests; however, it does not remove the need for ultrasound or other follow up tests. Another study has looked at the prognostic value for fetal outcome when CA125 and beta hCG levels are used together 18 . At present, however, the role of hormone measurement in the clinical management of early pregnancy failure and prognosis of fetal viability is extremely limited.…”

“…However, it is a subjective ® nding especially during early pregnancy and, in our experience, unreliable. Scarpellini et al (1995) em phasised the high serum levels of CA125 in patients with threatened abortion. There is similar interest in the evaluation of serum CA125 in current research studies.…”

“…Integrity of the gestational sac can be detected at 33± 35 days while the cardiac activity can be demonstrated as early as 40 days from last period using transvaginal scanning. Serum CA125, which is a monoclonal antibody ® rst developed by Bast and his colleagues (1983) against epithelial ovarian cancers, was reported by Scarpellini et al (1995) to be at higher levels in the sera of women with threatened abortion especially when associated with negative outcomes. They suggested that the value o f C A 125 c ould b e a progn ostic factor for the pregnancy outcome in patients with threatened abortion.…”

“…This high level is likely due to tropho-decidual origin of this marker and invasion of deciduas by chorionic villi. This decidual disruption is associated with vaginal bleeding (16)(17).…”
