Serum ghrelin concentrations in patients with chronic renal failure undergoing dialysis

Abstract: These results suggest that PD is accompanied by a striking decrement in baseline ghrelin concentrations in comparison with values found both in HD and control patients. Further studies are necessary to determine mechanisms involved in ghrelin regulation in uraemic patients.

“…Several studies also demonstrated no correlation between ghrelin levels and BMI in PD patients. [9,11,28] In these reports, especially, a relationship between ghrelin and adipose tissue has been reported. Ghrelin stimulates adipogenesis, and a reduction in ghrelin levels in PD patients was accompanied by a decrease in lean body mass and an increase in both fat mass and truncal fat mass.…”

“…A previous study suggested that plasma ghrelin concentrations are remarkably elevated in chronic renal failure (CRF) patients regardless of modality of therapy. [9] However, another report showed that ghrelin concentrations were strikingly lower in PD patients than patients on hemodialysis (HD) or predialysis patients. [10] In a pediatric population-based study, plasma ghrelin levels were similar in children on PD compared with CRF patients on conservative treatment.…”

“…Although ghrelin is degraded in the kidney, its levels are not uniformly increased in CKD patients [15]. Some studies with dialysis patients have demonstrated elevated ghrelin levels, whereas other studies have shown normal or even low ghrelin levels in CKD patients [16,17]. A possible explanation for these seemingly contradicting findings is that only UAG levels were elevated in the CKD patients.…”

“…Thus, measuring blood ghrelin level and determining its relationship with other important factors provide some valuable information about the role of ghrelin in the pathophysiology of various diseases such as type 2 diabetes, which is the main cause of chronic kidney failure (9). Moreover, with the decrease of kidney function, the ghrelin serum level may increase, up to 2-3 times higher in patients suffering from stage five of chronic kidney failure compared to healthy individuals emphasizing the important role of kidneys in the destruction of ghrelin (12)(13)(14). The high blood levels of ghrelin could entail the increase in the micro-and macro-vascular complications which may lead to ESRD (14).…”
