Back ground: Type 2 Diabetes mellitus is state of uncompensated insulin resistance. But what actually predisposes to it, is still a controversy. Obesity is often marked as a precondition for such insulin resistance. But the degree of association and mode of pathogenesis are grossly debated. Methods:Here we explored the correlation of physical parameters for obesity like BMI (Body Mass Index), abdominal circumference, waist-hip ratio and the adipocytokines like leptin and adiponectin with the degree of insulin resistance (HOMA-IR score) in 167 patients of type 2 Diabetes mellitus. Result:We found significant positive correlation between BMI, abdominal circumference, waisthip ratio and insulin resistance. Leptin showed significant positive correlation with all the said physical parameters. But, adiponectin showed a negative correlation with abdominal circumference, waist-hip ratio and fasting insulin. Leptin-adiponectin ratio showed a significant positive correlation with insulin resistance. Conclusion:Leptin shoots up with increased adiposity where as adiponectin gets reduced in central obesity which may contribute to insulin resistance. Though neither of leptin and adiponectin showed any significant correlation with insulin resistance individually, a significant positive correlation was found between leptin and adiponectin ratio and HOMA -IR score. Therefore, the ratio can be an authentic predictor of insulin resistance as it takes into account the interplay of both of the adipocytokines for its causation.

Introduction: The risk of developing insulin resistance and metabolic syndrome is particularly high in central obesity. In this study we evaluated the effects of fat distribution and some adipokines on insulin resistance in prediabetic patients. Material and methods: Eighty-seven age-and sex-matched patients were divided into three groups according to their 75-gram oral glucose tolerance test results as follows: impaired fasting glucose group, impaired glucose tolerance group, and normal glucose tolerance group. Fasting insulin levels were measured. Homeostatic model assessment of insulin resistance was calculated. Body fat mass measurements were assessed by bioelectric impedance analyser and abdominal fat thicknesses (subcutaneous, visceral, and preperitoneal) by ultrasonography. The fasting serum levels of several adipokines [adiponectin, leptin, resistin, vaspin, visfatin, retinol-binding protein-4 (RBP-4), tumour necrosis factor-a (TNF-a)] were measured by ELISA method. Results: The mean body mass index, fat mass measurements, and abdominal fat thicknesses of the groups were similar. There were no differences between groups in terms of the mean fasting insulin, vaspin, RBP-4, leptin, resistin, and TNF-a. In comparison of the prediabetic and normal groups, the levels of adiponectin (p < 0.001) and visfatin (p < 0.001) were lower in the prediabetic group. Furthermore, we found that high body mass index (p < 0.01) and fat mass (p < 0.01) and low adiponectin (p < 0.05) levels have roles in the development of insulin resistance in the prediabetic group. Conclusions: We suggested that in the prediabetic period not only obesity but also decreased adiponectin levels play some role in the pathogenesis of insulin resistance. Introduction: Very few systematic studies are done during the onset and progression of metabolic syndrome in suitable animal models. In this paper we present the effect of High-Fat Simple Carbohydrate (HFSC) feed on the metabolic hormones in C57BL/6J mice to understand the sequence of events leading to impairment of glucose homeostasis. Material and methods: One-month-old male C57BL/6J mice were fed with control (C group) and HFSC (T group) feed (n = 30 each) respectively for five months. The glucose tolerance was studied by Oral Glucose Tolerance Test (OGTT) whereas serum insulin and leptin were quantified using ELISA kits, and serum cortisol was quantified using CLIA kits. Results: Insulin resistance index and HOMA-IR levels were higher in the mice of group T as compared to age-matched mice of group C within one month and significantly higher after and five months of feeding. The total area under the glucose tolerance test curve (AUC) and the insulin curve (AUC ins) was found to significantly increase in the mice of T group as compared to the mice of C group as early as two months of feeding and was elevated after 5 months post feeding. Comparison of the Matsuda index revealed that pancreatic beta cell function was significantly lower in mice of T group as compared to mice of C group by five months of feeding. Leptin levels fluctuated during the 1 st -4 th month and by the 5 th month significant hyperleptinaemia was detected. There was no significant change in cortisol levels in mice of group T as compared to mice of group C after five months of feeding. Conclusions: HFSC feed induces insulin resistance by the first month and progressively impairs glucose tolerance, resulting in hyperleptinaemia by the fifth month in male C57BL/6J mice.
