Severe aortic regurgitation from aortic valve perforation in Libman‐Sacks endocarditis: Incremental value of 3D TEE

Abstract: A 43-year-old woman with stroke and systemic lupus erythematosus was found to have severe eccentric aortic regurgitation. Three-dimensional transesophageal echocardiogram further defined the origin of the regurgitant jet through a perforation in the left coronary cusp next to a mass with a tiny stalk attached to the left coronary cusp.

“…Since cropping planes are utilized to obtain images from any direction, 3D TOE has the advantage of providing perspectives that 2D alone cannot, allowing comprehension of the entire morphology of the vegetation (Figure 1, Video S1A-C). According to several studies [16][17][18][19] and case reports, [20][21][22][23][24] 3D TOE is superior in the assessment of suspected IE due to its ability to provide visual representation of the leaflets in an en-face view.…”

“…Measures true size of vegetations while avoiding underestimations caused by 2D TOE 16,17,28 Improved morphological characterization of the vegetation 16,23 More accurate determination of embolic risk 16,17 Useful in patients with PV (mechanical, biological or TAVR), as 3D TOE may enable the identification of vegetations that are not apparent with 2D TOE 29,30 Enhanced abscess visualization 21,[29][30][31][32][33] Improved characterization of the anatomical relationship between the periannular extensions and the proximal coronary arteries 29 Abscess Improved characterization of paraaortic abscesses in PV IE 31 Better visualization of pseudoaneurysms 24 Pseudoaneurysm Possible additive value of TI, for increasing the depth perception Perforation Increased capacity to visualize perforations due to visualization of the leaflets en face 22,24 Quantification of multiple regurgitant jets…”
