Severe Colitis Associated with both Epstein-Barr Virus and Cytomegalovirus Reactivation in a Patient with Severe Aplastic Anemia

Abstract: Epstein-Barr virus (EBV) and cytomegalovirus (CMV) are members of the herpesvirus family and common causes of viral infection in humans. CMV infection of the gastrointestinal tract occurs mainly in immunocompromised individuals, on the other hand EBV infection and reactivation involving the gastrointestinal tract is very rare. A 56-year-old man was diagnosed with severe aplastic anemia and treated with antithymocyte globulin (ATG) and cyclosporine (CSP). After 2 years of ATG/CSP therapy, he suddenly started pa… Show more

“…Cytomegalovirus (CMV) and Epstein Bar Virus (EBV), are prevalent among HIV-infected individuals, and can replicate within the intestinal mucosa causing severe diseases in the setting of immune suppression [12, 13]. The introduction of antiretroviral therapy (ART) reduced the incidence of AIDS-related viral colitis [14].…”

“…Presently, there are few studies that conclusively demonstrate a direct link between viral infections and intestinal inflammation. Most of these studies show only an observational relationship between the induction of intestinal inflammation by viruses and the onset of IBS or IBD in people [ 284 ]. Most studies examined coincidental associations between the presence of IBD in patients, the existence of viral pathogens and their remnants (i.e.…”

“…Therefore, local and mucosal viral infection or re-activation of HHV-6 and EBV should be taken into consideration, considering that 22 of 64 patients (34.4%) with underlying diseases showed a negative PCR result in the colon mucosa. Indeed, HHV-6 colitis [15] or severe colitis associated with EBV and CMV reactivation [8] was reported.…”

“…To make an exact diagnosis of these viral infections, viral genome analysis by polymerase chain reaction (PCR) in individual specimens has been performed. The specimens include cerebrospinal fluid (CSF) for meningitis/encephalitis by HSV-1 [1] and HSV-2 [2],[3], peripheral blood and bronchoalveolar lavage (BAL) fluid [4] for CMV pneumonia [5],[6] and colitis [7],[8], peripheral blood or blister fluid for herpes zoster/varicella [9][11] or herpes simplex, and urine for hemorrhagic cystitis [12],[13]. Generally, these specimens can be easily obtained and subjected to PCR assay.…”
