Severe Pulmonary Mycobacterium abscessus Cases Due to Co-Infection with Other Microorganisms Well Treated by Clarithromycin and Sitafloxacin in Japan

Abstract: Background: Mycobacterium abscessus frequently causes severe infections, yet its pathophysiological features and treatment regimens have not been established. Case Report: We present five cases of severe pneumonia due to Mycobacterium abscessus infection in Japan. All cases were diabetic patients, with possible acceleration to pneumonia due to co-infection with other microorganisms. However, following a short period of hospitalization and combination therapy with intravenous imipenem/cilastatin and amikacin, a… Show more

“…Analysis of its mechanism may suggest that clarithromycin has an anti-inflammatory effect. When clarithromycin enters the body, it can penetrate the cell wall of bacteria, inhibit the synthesis of bacterial ribosomal 50 S subunit protein, and finally inhibit the production of bacteria to achieve the purpose of anti-inflammatory [ 11 ]. After the inflammation is reduced or eliminated, the erythema fades, the papules subside, and the skin condition significantly improves.…”

“…Importantly, sitafloxacin exhibits antibacterial activity against M. tuberculosis and M. avium both in vitro and in vivo (Tomioka et al, 1999;Sato et al, 2003;Asakura et al, 2019). Cases of M. abscessus infection successfully treated with a drug combination that included sitafloxacin were reported (Oka et al, 2021;Takano et al, 2021). The efficacy of sitafloxacin alone in treating M. abscessus infections, however, remains to be evaluated.…”

“…massiliense ) activity that was comparable to clarithromycin, and which tended to be greater than moxifloxacin albeit without reaching statistical significance. Notably, five severe M. abscessus cases recently treated with a combination of clarithromycin and sitafloxacin achieved good clinical and microbiological outcomes ( Takano et al, 2021 ).…”

“…49,50 In the case of CF patients, M. abscessus infection may also be exacerbated by polybacterial lung infections with other microorganisms, such as P. aeruginosa and/or S. aureus. 46 In this context of coinfection, M. abscessus would be missed in the absence of effective microbiological surveillance, whereas it is of great importance to accurately detect its presence in patient's sputum in order to adapt the antimicrobial treatment. 22,49 Toward the generation of new antibacterial leads, the CyC analogues are not only nontoxic for mammalian cells, but above all represent a novel class of selective and efficient multitarget inhibitors leading to the arrest of M. abscessus extracellular and/ or intracellular growth through the inhibition of various mycobacterial enzymes involved in important physiological processes.…”
