Sex Differences in HIV Infection: Mystique Versus Machismo

Abstract: Biological sex is a determinant of both susceptibility to and pathogenesis of multiple infections, including HIV. These differences have effects on the spectrum of HIV disease from acquisition to eradication, with diverse mechanisms including distinct chromosomal complements, variation in microbiota composition, hormonal effects on transcriptional profiles, and expression of different immunoregulatory elements. With a comparative biology approach, these sex differences can be used to highlight protective and d… Show more

“…Importantly, many of these lifestyle factors are enriched among PWH [11][12][13][14]. Residual immune activation and inflammation, and some other key drivers, may differ between men and women [15], racial/ethnic groups [16], and geographic regions [17] and are likely to be influenced by genetic and environmental factors as well. The list of candidate drivers described here is likely incomplete and requires further study in PWH and uninfected populations to identify root drivers and their relative importance in different settings.…”

“…In COVID-19 pandemic we have been able to clearly observe sex differences in the immune response to the virus, and how these translate into clinical outcomes (195). Differences in the acquisition of HIV and in disease pathogenesis between men and women have long been also reported in epidemiological studies (196,197).…”

“…Women are at a higher risk of HIV seroconversion when engaging in heterosexual sexual behavior than men. This may be in part due to inflammation at the cervico-vaginal mucosa lowering the barrier to HIV infection [33]. Although in early infection, women seem to have slightly lower viral loads, disease progression is comparable in men and women.…”
