Objectives: To review the causes and management of penile fracture and to compare between surgical and conservative management as well as immediate and delayed interventions in terms of overall and specific complications. Methods: A search of all reported literature was conducted for all articles reporting on the management and outcomes of penile fractures. Full texts of relevant articles were obtained and screened according to the inclusion criteria. Outcomes measures were numbers of patients receiving surgical or conservative management, aetiology of fracture, length of admission, complications as well as the specifics of diagnostic approaches and operative management. Data was collated and where possible meta-analysed using Revman software. Results: A total of 58 relevant studies involving 3,213 patients demonstrated that intercourse accounts for only 48% of cases with masturbation and forced flexion accounting for 39%. Meta-analysis shows that surgical intervention was associated with significantly fewer complications vs. conservative management (p < 0.000001). Surgical intervention results in significantly less erectile dysfunction (ED), curvature and painful erection than conservative management. There was no significant difference in the number of patients developing plaques/nodules (p = 0.94). Meta-analysis shows that overall early surgery is preferable to delayed surgery but that rates of ED are not significantly different. Discussion: Early surgical intervention is associated with significantly fewer complications than conservative management or delayed surgery. The combined outcome of rapid diagnosis by history and clinical examination and swift surgical intervention is key for reconstruction with minimal long-term complications.

Introduction: Fracture of the penis is a rare urological emergency which occurs as a result of abrupt trauma to an erect penis. Immediate surgical repair is the standard of care and is superior to nonoperative management due to excellent long term outcomes. A large percentage of the patients present late for treatment out of fear or embarrassment. We report our series of patients who presented late and underwent delayed repair. We also report the long term outcome in these patients. Materials & Methods:The data was retrospectively collected from hospital records, which included: detailed history, symptoms, type of relationship, mechanism of trauma, sexual position, clinical fi ndings at physical examination, imaging results, presence of urethral injury, outcomes, and long-term complications regarding sexual and voiding functions.Results: Twenty two patients presented to our hospital casualty/emergency services following penile fracture and underwent delayed surgical repair. Heterosexual inter-course was the most common cause of fracture (18 patients, 81.81%). The mean time duration between injury and presentation was 48.77±33.56 hours. The mean time duration between presentation and surgical intervention was 4.31±1.37 hours. MRI was done in 6 cases with positive predictive value of 100%. Seventeen (77.27%) patients experienced erections during the post-operative period in the hospital. At the end of 1 year follow-up all the 22 patients had been having sexual intercourse.Conclusions: Our data suggests that, patients with penile fractures undergoing delayed repair have preservation of erectile potency and overall sexual function is maintained.

ÖzetAmaç: Penil fraktür nedeni ile cerrahi tedavi uyguladığımız hastaların sonuç-larını değerlendirmeyi amaçladık. Gereç ve Yöntem: Bu çalışmaya 2007-2013 yılları arasında hastanemize penil fraktür nedeni başvuran ve erken cerrahi onarım yapılan toplam 19 erkek hasta dahil edildi. Hastaların kayıtlarından operasyon öncesindeki yaş, uluslararası erektil fonksiyon indeksi'nin ilk 5 sorusu (IIEF-5) skoru, penil fraktür etiyolojisi, penil fraktür oluşumundan kliniği-mize başvurduğu ana kadar geçen süre ve hospitalizasyon süresi retrospektif olarak elde edildi. Operasyon sonrası dönemde, IIEF-5 skoru, penil his kaybı, ağrılı penil ereksiyon ve penil kurvatur gelişimi değerlendirildi. Bulgular: Hastaların ortalama yaşı 37.8 yıl idi. Ortalama takip süresi 33.4 ay idi. Ortalama cerrahiye kadar geçen süre ve hastaların operasyon öncesindeki ortalama IIEF-5 skoru sırası ile 11.1 saat ve 22.7 idi. Operasyon öncesindeki dönemde 1 hastada orta-hafif derecede erektil disfonksiyon (ED), 3 hastada hafif ED var idi. On beş hastada ED saptanmadı. Operasyon sonrasında-ki dönemde ortalama IIEF-5 skoru 19.3 idi. Operasyon öncesi ve sonrası dö-nemdeki IIEF-5 skorları arasında istatistiksel olarak anlamlı fark mevcut idi (p=0.01). Operasyon sonrasındaki dönemde 5 (%26.3) hastada penil kurvatür, 4 (%21.05) hastada ağrılı ereksiyon ve 2 (%10.52) hastada penil his kaybı saptandı. Sonuç: Sonuçlarımız ışığında, penil fraktür nedeni ile erken cerrahi tedavi yapılan hastaların operasyon sonrasındaki dönemde gelişebilecek olası penil kurvatur, ağrılı ereksiyon ve ED açısından iyi bilgilendirilmesi gerektiğine inanıyoruz.
