Short Bowel Syndrome

Abstract: The short bowel syndrome is a symptom complex that occurs in adults who have less than 200 cm of jejunum-ileum remaining after intestinal resection. Similar symptoms are observed in infants and children following massive bowel resection or congenital anomalies and in individuals with longer segments of intestine with severe mucosal disease. Initial care should focus on a thorough excision of nonviable bowel, an exact measurement of the remaining viable bowel, placing all intestine in continuity at the initial … Show more

“…Despite several treatment options, such as long-term parenteral nutrition, intestinal rehabilitation, various intestinal lengthening procedures, and, more recently, small bowel transplantation, the outcomes with current therapies are still not optimal for patients with SBS [1][2][3][4]. Preserving the distal ileum with the ileocecal junction in patients undergoing massive small bowel resection is associated with better surgical outcomes; however, the adaptive mechanisms occurring in the distal ileum for improving nutrient absorption, and especially for proteins as peptides, has not been characterized.…”

“…Extensive operative resection of the small intestine can be imperative in the management of several pathologic conditions but may result in the devastating malabsorptive state of "short bowel syndrome (SBS)" [1][2]. A similar state can follow traumatic loss of intestinal surface area, also leading to SBS.…”

“…Wilmore i wsp. [40] oraz Byrne i wsp. [41] zaobserwowali, że glutamina podawana dojelitowo (dawki odpowiednio 30 g/dobę i 0,6 g/kg/dobę wraz z podawanym podskórnie hormonem wzrostu w ilości 0,03-0,14 mg/kg/dobę) w stosunku do żywienia dojelitowego bez glutaminy poprawia wchłanianie składników odżywczych (o 39%) i powoduje zmniejszenie ilości wydalanego stolca (o 33%).…”
