Shortest Apposition Length at the First Postoperative Computed Tomography Angiography Identifies Patients at Risk for Developing a Late Type Ia Endoleak After Endovascular Aneurysm Repair

Abstract: Imaging surveillance following endovascular aneurysm repair (EVAR) is strictly recommended. This study investigates the value of endograft apposition and position relative to the aortic neck on the first postoperative computed tomography angiography (CTA) in determining patients at risk for a late type Ia endoleak (T1aEL). Materials and Methods: Patients with a T1aEL after the first postoperative CTA were selected from a consecutive database and matched with uncomplicated controls. Endograft apposition and pos… Show more

“…Inclusion criteria were based on a previous study by this research group regarding apposition on the first postoperative CTA, with largely the same patients [ 13 ]. The late T1aEL group included patients with a first post-EVAR CTA (<90 days post-EVAR) without a type 1 (or type 3) endoleak, who developed a T1aEL thereafter.…”

“…All CTAs were analysed in 3mensio 10.1 software (Pie Medical Imaging BV, Maastricht, The Netherlands). The entire CTA measurement protocol has been published [ 13 ]. On each preoperative CTA, the neck diameter, intended oversizing, neck length, infrarenal/suprarenal angulation, neck thrombus, neck calcification, neck shape, and maximum aneurysm diameter were measured.…”

“…The T1aEL group was composed of patients from 2 previous studies, to include a sufficiently large number of patients with a T1aEL. 19,20 These patients were treated between 2005 and 2017 in the St. Antonius Hospital, Nieuwegein (n=10), Royal Oldham Hospital, Manchester (n=2), and Catharina Hospital, Eindhoven (n=10). Fortyone patients originated from the ODYSSEUS study group (Figure 1).…”
