Should we diagnose and treat distal deep vein thrombosis?

Abstract: Ultrasound series report that isolated distal deep vein thrombosis (DVT), also known as calf DVT, represents up to 50% of all lower-limb DVTs and, therefore, is a frequent medical condition. Unlike proximal DVT and pulmonary embolism, which have been studied extensively and for which management is well standardized, much less is known about the optimal management of isolated calf DVT. Recent data arising from registries and nonrandomized studies have suggested that most distal DVTs do not extend to the proxima… Show more

“…As distal DVT is frequent and accounts for up to half of all lower limb DVT, this is an important knowledge gap. Furthermore, whether distal DVT needs to be treated with therapeutic anticoagulation is still debated and anticoagulant treatment is not routinely recommended in international guidelines . Thus, in clinical practice, not all patients with distal DVT are offered therapeutic anticoagulation, which could have an impact on their risk of developing future PTS.…”

“…Multivariate analysis showed a statistically significant ( p value = 0.019) reduction in DVT incidence between patients who received only mechanical thromboprophylaxis (10.05%) and patients who received additional chemoprophylaxis (3.42%). However, most DVT observed in our study were distal DVTs, which have limited clinical significance as they are less strongly associated with PE compared to proximal DVT [ 18 , 19 ].…”

“…The clinical significance and management of distal DVT is another area of controversy [ 26 ]. Unlike proximal DVT and PE, which have been extensively studied, with anticoagulation being the mainstay of treatment, much less is known about the optimal management of distal DVT [ 19 , 27 ]. While some advocate anticoagulation to prevent proximal extension and PE, others favor a more conservative approach of close monitoring and ultrasound surveillance as distal DVT are less likely to extend to proximal veins and lead to PE [ 18 , 19 , 28 ].…”

“…[16,17] Nevertheless, the bene t of treating patients with isolated calf DVT remains uncertain and controversial. [17][18][19][20][21][22][23] In light of this ongoing controversy, we sought to investigate the prevalence and clinical characteristics of isolated calf DVT on complete duplex ultrasound examination as the initial test in patients with suspected DVT.…”
