Sildenafil Increases Digital Skin Blood Flow During All Phases of Local Cooling in Primary Raynaud's Phenomenon

Abstract: Digital skin vasoconstriction on local cooling is exaggerated in primary Raynaud's phenomenon (RP) as compared with controls. A significant part of such vasoconstriction relies on the inhibition of the nitric oxide (NO) pathway. We tested the effect of the phosphodiesterase 5 (PDE5) inhibitor sildenafil, which potentiates the effect of NO, on skin blood flow. We recruited 15 patients with primary RP, performing local cooling without sildenafil (day 1), after a single oral dose of 50 mg (day 2), and after a dos… Show more

“…There is a continuous search for new car diovascular indications, and in animal studies PDE5 inhibitors have proven to be protective in congestive heart failure, ischaemic heart disease 199,200 , peripheral artery disease 201 and doxorubicin cardiomyopathy 202 . Moreover, in smaller clinical trials, PDE5 inhibitors have been shown to attenuate vasoconstriction in Raynaud phenomenon 203 .…”

“…e subjects were asked not to drink any alcohol or not to use any other stimulants during the testing period. Women were excluded from the study because differences in the hormone economy induce differences in regular flows and the provoked ones [30]. e participants were randomly divided into two groups with 24 people in each of them.…”

“…This leads to increased endogenous nitric oxide levels, with subsequent vasodilation. 32 Several PDE5 inhibitors have been proposed in the treatment of SSc-associated vasculopathy, and in the literature three uncontrolled pilot studies have highlighted the beneficial effect of sildenafil on DU healing and its safety. [33][34][35] A meta-analysis of three RCTs investigated various selective PDE5 inhibitors:sildenafil 50 mg twice a day, 36 modified-release sildenafil 100 mg/day increased up to 200 mg/day, 37 and tadalafil 20 mg on alternate days.…”
