Silica exposure and systemic vasculitis.

Abstract: Work in Department of Energy (DOE) facilities has exposed workers to multiple toxic agents leading to acute and chronic diseases. Many exposures were common to numerous work sites. Exposure to crystalline silica was primarily restricted to a few facilities. I present the case of a 63-year-old male who worked in DOE facilities for 30 years as a weapons testing technician. In addition to silica, other workplace exposures included beryllium, various solvents and heavy metals, depleted uranium, and ionizing radiat… Show more

“…Activation of the immune system by crystalline silica deposited in the alveoli is linked with an increased risk of autoimmune diseases in the silica-exposed patients: systemic sclerosis (scleroderma), rheumatoid arthritis or Caplan's syndrome, systemic lupus erythematosus, systemic vasculitis (3,39,(52)(53)(54)(55). Elevated frequencies were also recorded for sarcoidosis, dermatomyositis, dermatopolymyositis, autoimmune hemolytic anemia (3,39,52).…”

“…W p贸藕niejszych latach opisano liczne przypadki RPGN u chorych nara偶onych na krzemionk臋 -jednocze艣nie stwierdzano u nich wyst臋powanie przeciwcia艂 ANCA (g艂贸wnie anty-MPO) [50,51]. Przedstawiane przypadki dotyczy艂y g艂贸wnie m臋偶czyzn powy偶ej 50. roku 偶ycia, zwykle z kilkudziesi臋cioletnim okresem nara偶enia na krzemionk臋.…”

“…W wynikach biopsji nerki opisywano typowe zmiany z p贸艂ksi臋偶ycami i brakiem z艂og贸w w badaniu immunofluorescencyjnym. W surowicy krwi stwierdzano przeciwcia艂a ANCA, a na podstawie obrazu klinicznego rozpoznawano mikroskopowe zapalenie naczy艅 [50,51]. Pod wzgl臋dem danych demograficznych (g艂贸wnie m臋偶czy藕ni powy偶ej 50. roku 偶ycia), objaw贸w klinicznych, wynik贸w biopsji nerek i bada艅 immunologicznych by艂y to wi臋c przypadki typowe dla AAV [48].…”

“…Lung cancer is also considered to be associated with silicosis and the International Agency for Research on Cancer (IARC) categorized crystalline silica as a causative of lung cancer (Cocco, 2007;IARC, 1997;Pelucchi, 2006). In addition, it is well known that silicosis patients (SILs) often experience complications due to autoimmune diseases (Shanklin & Smalley, 1998;Steenland & Goldsmith, 1995;Uber & McReynolds, 1982) such as rheumatoid arthritis (known as Caplan syndrome) (Caplan, 1959(Caplan, , 1962, systemic lupus erythematosus (SLE) (Bartsch, 1980;Yamazaki 2007), systemic scleroderma (SSc) (Barnadas, 1986;Cowie, 1987Haustein, 1990Haustein & Anderegg, 1998;Sluis-Cremer, 1985) and antineutrophil cytoplasmic autoantibody (ANCA)-related vasculitis/nephritis (Bart暖nkov谩, 2006;Mulloy, 2003;Tervaert, 1998). Silica-induced dysregulation of autoimmunity has been thought to be caused by the adjuvant effect of silica (Cooper, 2008;Davis, 2001, Parks, 1999.…”
