The role of advanced airway management (AAM) in cardiopulmonary resuscitation (CPR) is currently debated as observational studies reported better outcomes after bag-mask ventilation (BMV), and the only prospective randomized trial was inconclusive. Adherence to CPR guidelines ventilation recommendations is unknown and difficult to assess in clinical trials. This study compared AAM and BMV with regard to adherence to ventilation recommendations and chest compression fractions in simulated cardiac arrests. A total of 154 teams of 3–4 physicians were randomized to perform CPR with resuscitation equipment restricting airway management to BMV only or equipment allowing for all forms of AAM. BMV teams ventilated 6 ± 6/min and AAM teams 19 ± 8/min (range 3–42/min; p < 0.0001 vs. BMV). 68/78 BMV teams and 23/71 AAM teams adhered to the ventilation recommendations (p < 0.0001). BMV teams had lower compression fractions than AAM teams (78 ± 7% vs. 86 ± 6%, p < 0.0001) resulting entirely from higher no-flow times for ventilation (9 ± 4% vs. 3 ± 3 %; p < 0.0001). Compared to BMV, AAM leads to significant hyperventilation and lower adherence to ventilation recommendations but favourable compression fractions. The cumulative effect of deviations from ventilation recommendations has the potential to blur findings in clinical trials.

Use of Cognitive Aids: Results from a National Survey among Anaesthesia Providers in France and Canada

Introduction. The use of cognitive aids (CAs) during critical events is thought to be useful. However, whether CAs are known and used by French and Canadian anaesthesia providers is not clear. Methods. A survey was emailed to French and Canadian anaesthesia providers in 2017 through their respective national societies. It consisted of 23 questions about the participants’ demographics and their knowledge, use, and impact of CAs. A second survey was sent to French simulation centres. Results. 912 responses were recorded in France and 278 in Canada (overall response rate: 7% and 11%, respectively). Among the respondents, 700/899 in France (78%) versus 249/273 (91%) in Canada were familiar with the concept of cognitive dysfunction during a crisis and 501/893 (56%) in France versus 250/271 (92%) in Canada knew the concept of CAs. Amongst those respondents who knew about CAs, 189/492 (38%) in France versus 108/244 (44%) in Canada stated that they had already used a CA in real life and 225/493 (45%) in France versus 126/245 (51%) in Canada had received training in their use. Simulation was the principal modality for training in 150/225 (67%) of cases in France versus 47/126 (37%) in Canada. Among the 28/50 French simulation centres which responded (2018 January), 27 organised sessions in anaesthesia and 22 used CAs. Conclusion. CAs were better known in Canada than in France, but their actual use in real life was low in both countries. Simulation appears to play a potentially important role training anaesthesia providers in the use of CAs.

Objetivo: Analisar as publicações científicas relacionadas à atuação do Enfermeiro na prevenção das toxemias gravídicas.Método: Revisão integrativa da literatura. As buscas das evidências científicas foram realizadas nas bases de dados: Literatura Latino-Americana e do Caribe em Ciências da Saúde, Base de Dados de Enfermagem e Medical Literature Analysis and Retrieval System Online via Biblioteca Virtual da Saúde e Scientific Electronic Library Online, utilizando-se dos termos de busca: Assistência de Enfermagem, Cuidados de Enfermagem, Enfermagem, Pré-eclâmpsia e Eclâmpsia, com auxílio dos operadores booleanos “AND” e “OR”. Resultado: Após a seleção, leitura e filtragem, identificaram-se 10 estudos para análise, que foram agrupados por similaridade semântica e discutidos em categorias temáticas, sendo elas: Atuação do Enfermeiro na prevenção das toxemias gravídicas e Dificuldades encontradas na atuação do Enfermeiro na prevenção das toxemias gravídicas. Conclusão: É notável que com a realização de um acompanhamento pré-natal de qualidade durante toda a gestação é possível detectar de forma precoce as alterações nos sinais clínicos causadas pela toxemias gravídicas e iniciar um tratamento adequado.
