Objective: To determine impact of various forms of intra nasal packings and trans-septal suturing (without nasal packing) on cardiopulmonary and metabolic milieu of patients undergoing nasal and paranasal sinus operations. Study Design: Randomized controlled trial. ( Ltr no: IERB-01/3/23/Trg dated 8 March 2023) Place and Duration of Study: Department of Otorhinolaryngology (ENT), Combined Military Hospital, Jhelum Cantonment, Pakistan, from Aug 2022 to Aug 2023. Methodology: Clinical and laboratory markers of cardio-pulmonary and metabolic assessment of 140 subjects were evaluated, who had undergone some form of intranasal packing procedure or trans-septal suturing (without nasal packing) following nose or paranasal sinus operations. Results: Ventilated nasal packs and completely avoiding nasal packing by performing trans-septal suturing have a significant impact on clinical parameters like resting heart rate, respiration, blood pressure, arterial blood pH and arterial blood partial pressure of carbon dioxide (PaCO2) in patients undergoing surgical operations of the nose and paranasal sinuses. Conclusion: We strongly recommend adopting less invasive and technically more advanced operative methodology and refining surgical expertise regarding septal splinting during rhinologic operative procedures, mitigating discomfort and antecedent morbidity in patients undergoing these sino-nasal surgical operations.

Nasal fractures are commonly treated using nasal packing as an internal splint after performing closed reduction. Although gauze and polyvinyl alcohol sponge (PVAS) are both commonly used for packing, few reports have compared their efficacy and safety in nasal fracture treatment. Therefore, in this study, the authors, aimed to retrospectively compare the efficacy and safety of gauze packing in 208 patients and PVAS packing in 225 patients who underwent closed reduction at our hospital. The proportions of patients requiring revision surgery did not differ significantly between the PVAS and gauze groups (P=0.627). However, in comparison to the gauze group, the PVAS group displayed significantly fewer instances of uncontrolled epistaxis following packing removal, as well as fewer occurrences of dropout and loss of packing (P=0.023, P=0.007, and P<0.001, respectively). The results of logistic regression analysis adjusted for confounding factors also showed that compared with the gauze group, the PVAS group was significantly less likely to experience packing dropout (odds ratio=0.13; 95% confidence interval, 0.06-0.29, P<0.001) for which younger and older age and allergic rhinitis were independent risk factors. In conclusion, both gauze and PVAS are effective packing materials as internal splints after closed reduction of nasal bone fractures. Specifically, PVAS utilization warrants consideration in pediatric and geriatric populations, individuals with allergic rhinitis, and those at elevated risk of bleeding.

Healing large Traumatic Tympanic Membrane Perforations Using Vaseline Gauze and Gelfoam Patching Alone

Objective The aim of this study was to evaluate the effect of vaseline gauze (VG) patching on the treatment of large traumatic perforation of tympanic membrane (TM). Material and methods 90 patients with traumatic perforation larger than 25% of the TM were randomly allocated into the control group of observation only, VG group, and Gelfoam patch alone group. The closure rate and closure time among the 3 groups were compared at 3 months. Results In total, 82 large traumatic perforations were analyzed in this study. The closure rates in the control, VG, and gelfoam patch groups were 84.6%, 100.0%, and 89.3%, respectively ( P = 0.637). Post-hoc multiple comparisons showed that the difference between the control and VG groups was significant ( P = 0.047), but the difference wasn’t significant between gelfoam alone and control groups ( P = 0.699) or VG groups ( P = 0.236). The mean closure times were 5.41 ± 1.47, 2.14 ± 0.93, and 3.00 ± 0.62 weeks for the control, VG, and gelfoam patch groups, respectively ( P < 0.001). Post-hoc multiple comparisons showed that the difference was significant between the control and VG groups ( P < 0.001) or gelfoam alone group ( P < 0.001) or VG and gelfoam groups ( P < 0.05). Conclusions VG improved the closure rate and shortened the closure time compared with observation only, which could be an effective patch material for repairing traumatic lager perforations in the outpatient setting, which is readily available and convenient.
