Simvastatin Attenuates Plaque Inflammation

Abstract: 18F-fluorodeoxyglucose PET visualized plaque inflammation and simvastatin attenuated it. The LDL-C-independent effects of simvastatin may participate in the beneficial effect. 18F-fluorodeoxyglucose PET has a potential for visually monitoring plaque inflammation and the therapeutic effectiveness of statins.

“…4). 18 F-FDG PET is increasingly used for monitoring response to therapy in a variety of malignancies (30), and a similar role for 18 F-FDG PET in inflammatory conditions is beginning to be explored (31)(32)(33). If 18 F-FDG uptake proves capable of accurate, early assessment of response to therapy in patients with Crohn disease, this information may enable modification or discontinuation of ineffective drug therapies that carry significant toxicities or expense.…”

“…Statins, for example, have been demonstrated to reduce both carotid atherosclerotic plaque burden 4,5 and plaque composition 6 on magnetic resonance (MR), as well as reducing vascular 18F-FDG uptake on PET. 7 Other medications, such as cholesterol ester transfer protein inhibitors, have by comparison failed to reduce 18F-FDG activity mirroring the similarly disappointing effects of these drugs on clinical outcomes. 8,9 The recent emergence of hybrid PET/MR systems offers major potential in this field, allowing combined, detailed assessment of large vessel plaque burden (MR), composition (MR), and activity (PET) at radiation doses considerably lower than possible…”
