Synchronous colorectal tumors that require surgical treatments are rare. Preliminary experience with concurrent single-incision laparoscopic right hemicolectomy and sigmoidectomy for synchronous carcinoma is reported. A 61-year-old woman presented to our department for the close examination of a bloody stool. Colonoscopy revealed two masses in the right-sided transverse colon and sigmoid colon and another slightly elevated lesion in the transverse colon, and all biopsies from these three lesions demonstrated adenocarcinoma. Under the diagnosis of transverse colon cancers and sigmoid colon cancer, we performed simultaneous singleincision laparoscopic sigmoidectomy and right hemicolectomy. First, a lap protector was inserted through a 2.5 cm transumbilical incision. Three 5 mm ports were placed in the lap protector. We successfully performed sigmoidectomy and right hemicolectomy with lymph node dissection. The patient was discharged on the thirteenth postoperative day. Postoperative follow-up did not reveal any umbilical wound complications. SILS should be the treatment of choice for concurrent laparoscopic surgery for also the other diseases.

Simplifi ed transumbilical sleeve gastrectomy. Technique and surgical results in 100 patientsBackground: The use of transumbilical approach for sleeve gastrectomy has been recently reported, using different technique variations. Aim: To report the technique and surgical results of a transumbilical approach simplifi ed sleeve gastrectomy, using rigid instruments. Material and Methods: Ninety four women and six men, selected by a multidisciplinary team, underwent transumbilical sleeve gastrectomy. The operative technique involved a transumbilical incision, introduction of a SILS ® or GelPoint ® multiport, and a 5mm metallic accessory trocar laterally in the left fl ank. Rigid instruments were used in all patients. The greater curvature was dissected from 4-5 cm above the pylorus to the angle of His. Gastric transection was completed with a stapler, and calibrated with a 36 French tube advanced through the pylorus. Hemostasis of the staple line was carried out with metallic clips. A barium swallow was performed in ten randomly chosen patients, confi rming the correct tubular shape of the stomach. Results: Body mass index of operated patients ranged from 30 to 43 kg/m 2 . Mean operative time was 56.4 ± 16.7 minutes. During the early postoperative period, two patients had a hemoperitoneum, one had an antral leak and one had an intestinal perforation. No conversion to conventional laparoscopy or open technique was required. No patient died. The mean length of hospital stay was 2.3 ± 0.5 days. The cosmetic result was satisfactory for all patients. Conclusions: Transumbilical sleeve gastrectomy is a safe and feasible procedure with the reported technique. The insertion of an accessory 5mm trocar in the left fl ank simplifi es the procedure, allowing the use of rigid instruments.

20Лапароскопические технологии нашли широкое при-менение в хирургии рака ободочной и прямой кишки и являются одним из наиболее динамично развивающихся методов хирургического лечения. Производителями меди-цинских инструментов представлена целая серия техноло-гий, позволяющих выполнять миниинвазивные оператив-ные вмешательства при опухолевых заболеваниях толстой кишки. В настоящее время актуальным является решение вопроса о предпочтении применения той или иной мето-дики оперативного вмешательства у конкретного пациента. Цель -провести ретроспективный анализ результатов операций при раке ободочной и прямой кишки, выполненных по методике единого лапароскопического доступа и по стандартной лапароскопической технологии. Материал и методы. В исследование включили 110 лапароскопических операций, выполненных в Лечебно-реабилитационном центре за пери-од с марта 2009 г. по декабрь 2013 г. Из них 44 операции выполнены по методике единого лапароскопического доступа (1-я группа) и 66 -по стандартной лапароскопической технологии (2-я группа). Статистически значимых различий между группами по полу, возрасту, индексу массы тела, локализации, размеру и стадии опухоли не выявлено. Чаще в качестве порта для единого доступа использовали SILS-порт. Удаление препарата и формирование анастомоза осуществляли через трансумбиликальный доступ. При стандартных лапароскопических операциях препарат удаляли через поперечный доступ над лоном. Результаты. Статистически значимых различий по основным оцениваемым показателям не выявлено. Тяжелые осложнения выявлены у 16 и 12% пациентов соответственно. Средняя продолжительность послеоперационного койко-дня составила 7 и 8 сут соответственно. В каждой группе отмечено по одному летальному исходу. Удовлетворенность кос-метическим эффектом (5 и 4 балла) в 1-й и 2-й группах составила 98,6 и 96,4% соответственно. Возращение к обычному образу жизни в течение 3 нед отмечено у 79,0 и 76,8% больных соответственно. Заключение. Применение технологии единого доступа в хирургии рака ободочной и прямой кишки позволяет добиться результатов, не уступающих результатам стандартных лапароскопических вмешательств. Дальнейшие сравнительные исследования позволят прояснить роль этой технологии в комплексном лечении пациентов.
