Single incision laparoscopic surgery transabdominal pre-peritoneal hernia repair - case report.

Abstract: Single incision laparoscopic surgery (SILS) minimizes perioperative trauma and allows surgical procedures without a visible scar. Here we present a case in which the transabdominal pre-peritoneal method was used in inguinal hernioplasty using a SILS port. The presented case is part of a new trend in surgery to minimize operative trauma. In 2009, when the operation was conducted, it was an innovative procedure.

“…Pain and the risk of incarceration are the most important indications for operative treatment of inguinal hernia in children. In addition to conventional open techniques of inguinal hernia surgical repair, there have been many reports regarding the possibility of laparoscopic treatment of inguinal hernia since the 1990s [ 6 12 ]. In 2004, in Poland, Patkowski introduced a method of laparoscopic herniorrhaphy using his own technique.…”

“…However, we suggest that meticulous closure of the fascial defect may be needed in cases with large peritoneal and fascial defects or high-risk factors for TSH, even at 8-mm trocar or non-midline sites. Moreover, the trocar should be removed under direct vision and illumination to prevent any bowel entrapment or port removal and fascial defects at the port site [6, 7]. After TSH occurred, we changed the working port placement instead of using the previous bikini-line incision.…”

“…Series reports for SITAPP repair for inguinal hernias, too, are very few and interspersed with case reports. [ 11 12 19 20 21 ] Menenakos[ 13 ] reported on a patient with B/L inguinal hernia in whom they performed a SITAPP. The incision was supraumbilical and Triport access was used, however type of instruments has not been mentioned.…”
