The objective of this study is to report the current status on single-port laparoscopic surgery in gynecology. A systematic MEDLINE review of the English language literature from 2007 to 2011 was performed using the search term "single-port surgery," which contained information on single-port laparoscopic surgery in gynecology. Overall, 1,152 patients (26 studies) were included in the analysis. The operative time varied according to the type of procedure. The conversion rate to conventional laparoscopy was 1.99 %, and the conversion rate to laparotomy was 0.35 %. Postoperative complications were encountered in 1.48 % of cases. Different gynecologic procedures have been effectively performed by means of single-port laparoscopic surgery. The procedure seems to be feasible, reproducible, and safe. Reduced pain and improved cosmesis are supposed to be the potential advantages of these procedures, but it was not completely confirmed yet! Further studies are still necessary to establish the real benefits of this new surgical approach over the traditional multi-port laparoscopy.

: Background and Objective : To evaluate the efficacy and safety of 2 -port laparoscopic surgery for ovarian tumors and uterine leiomyomas. A conventional slip knot under laparoscopic surgery is useful, but it is difficult for beginners. Therefore, we developed new suture technique. Methods : Between January 2011 and December 2012, 38 patients underwent 2 -port total laparoscopic cystectomy (TLC) ; between January 2010 and December 2011, 45 patients underwent multiport (3 or 4 ports) TLC. Between January 2011 and December 2012, 25 patients underwent 2 -port laparoscopic myomectomy (LM) ; between January 2010 and December 2011, 34 patients underwent multiport (3 or 4 ports) LM. Surgery time, blood loss, postoperative length of stay and complications were retrospectively compared in each group. Results : No significant differences in surgery time, blood loss or postoperative length of stay were found between 2 -port TLC and multiport TLC. No significant differences were found in the aforementioned parameters between 2 -port LM and multiport LM. We showed here a new suture technique 'intra -abdominal suturing/extra -abdominal traction method' instead of conventional slip knot. Conclusions : We confirmed that 2 -port TLC and LM are less invasive than conventional multiport laparoscopic surgery ; furthermore, they are as safe as the conventional procedure. The new suture technique is easier than conventional slip knot and can applies sufficient tension to the suture knot for beginners.

In general, laparoscopic surgery leads to less scarring, less postoperative pain and more rapid healing rates compared to laparotomy (1,2) . Data are also available indicating that laparoscopic surgery has lesser complication rates compared to laparotomic surgery (3,4) . Although laparoscopic surgery is relatively safe, its causes unwanted and severe intestinal, bladder and vascular injury, requiring complicated equipment and a long learning curve limit is required for surgeons to master its use (5,6) . Most of the severe complications are related to trocar insertion (7,8) .
