Sinonasal risk factors for eustachian tube dysfunction: Cross‐sectional findings from NHANES 2011‐2012

Abstract: Background: Sinonasal pathology is frequently cited as a cause of eustachian tube dysfunction (ETD), despite a lack of evidence. Using a large, nationally representative sample, we investigated whether abnormal tympanometry, an objective marker of ETD, was associated with infectious, allergic, and exposure-related sinonasal risk factors. Methods:Relevant data were extracted from the 2011-2012 National Health and Nutrition Examination Survey (NHANES). Tympanometric types (A, B, and C) were determined using peak… Show more

“…Oftentimes, the assumption of the treating physician is that this symptom reflects eustachian tube dysfunction (ETD). ETD has been shown to be a comorbid condition in several diseases such as allergic rhinitis, nonallergic rhinitis, and chronic rhinosinusitis . The seven‐item Eustachian Tube Dysfunction Questionnaire (ETDQ‐7), which consists of seven questions, each ranked on a Likert scale ranging from 1 (no problem) to 7 (severe problem), has recently been validated and touted as having excellent discrimination for patients with ETD.…”

Whereas the maximum total score is 49, a score ≥14.5 has been reported to have 100% sensitivity and 100% specificity for ETD.…”

“…Many reports by Down et al and others underlined the effect of histamine and aeroallergen in blocking the ET, suggesting that allergic rhinitis causes or aggravates this condition [16,22]. Notifiable symptoms are exacerbated in allergy and pollen seasons [16]. Juszczak et al in 2018 supported with evidence the connection between allergic rhinitis and ETD [16].…”

“…Applying this tool in ET dysfunction is not yet well recognized systematically. A type C tympanogram reflects an ET dysfunction, which is a frequent pattern of subclinical middle ear malfunctions [8,16]. Although a negative middle ear tube pressure on tympanometry, especially when the peak is below − 100 daPa, can indicate an ET dysfunction, the diagnosis remains presumably clinical.…”
