In 2021, Lithuania faced the challenge of managing the crisis of new type of migration across the Belarusian border, which threatened the security of the country and its citizens. A broad set of inter-institutional collaboration strategies and methods were used among the public institutions to address this crisis. Such inter-institutional collaboration has been a multifaceted and complex process, involving both known and new good practices. The aim of this article is to provide insights into good collaborative practices between the Lithuanian national defence and internal affairs systems in managing the migrant crisis. Following the expert interview results, the study identified three good practices of inter-institutional collaboration: (i) the creation of a single dissemination channel for the crisis response services, which ensured that all actors were in the same information field; (ii) the delegation of representatives of the institutions to teams at strategic, operational and tactical levels; and (iii) inter-institutional information sharing ensured good situational awareness at an early stage of emergency management. Lessons learned from the collaboration between the institutions of the national defence and internal affairs systems can be applied to improve inter-institutional collaboration in other areas. The research results can be used to enhance the process of such collaboration in addressing current emergency management issues and serve as a basis for effective and smooth emergency preparedness and response where inter-institutional synergy is needed.

Background: Managing critical bleeding with massive transfusion (MT) requires a multidisciplinary team, often physically separated, to perform several simultaneous tasks at short notice. This places a significant cognitive load on team members, who must maintain situational awareness in rapidly changing scenarios.Similar resuscitation scenarios have benefited from the use of clinical decision support (CDS) tools. Study Design and Methods: A multicenter, multidisciplinary, user-centered design (UCD) study was conducted to design a computerized CDS for MT. This study included analysis of the problem context with a cognitive walkthrough, development of a user requirement statement, and co-design with users of prototypes for testing. The final prototype was evaluated using qualitative assessment and the System Usability Scale (SUS).Results: Eighteen participants were recruited across four institutions. The first UCD cycle resulted in the development of four prototype interfaces that addressed the user requirements and context of implementation. Of these, the preferred interface was further developed in the second UCD cycle to create a high-fidelity web-based CDS for MT. This prototype was evaluated by 15 participants using a simulated bleeding scenario and demonstrated an average SUS of 69.3 (above average, SD 16) and a clear interface with easy-to-follow blood product tracking.Discussion: We used a UCD process to explore a highly complex clinical scenario and develop a prototype CDS for MT that incorporates distributive situational awareness, supports multiple user roles, and allows simulated MT training. Evaluation of the impact of this prototype on the efficacy and efficiency of managing MT is currently underway.

In March 2020, the municipality of Oslo's Nursing Home Agency was hit by Norway's first COVID‐19 outbreak. Being responsible for a very vulnerable group, they had to deal with a situation never before encountered and of which they had very limited knowledge. In this study, we explored how situational awareness (SA) changed from a creeping to an urgent crisis. We undertook a case study of the Nursing Home Agency's top management during the initial period of the COVID‐19 pandemic (December 2019 through late March 2020). We conducted individual interviews with the management in charge of decisions. Thematic analysis yielded four main categories affecting SA: perception of event development, perception of available time, information, and cooperation and trust. We found that subjective experience of the geographical proximity of the crisis and subjective experience of time were essential in shaping SA. Perception of time was essential to the understanding of urgency, which was an important factor in reacting properly. Further, the perception of space was necessary for the crisis to be interpreted as critical. Time and space are objective factors but are perceived subjectively. Our model showed that the crisis must be perceived as urgent for proper actions to be decided upon.
