Soft tissue sealing around dental implants based on histological interpretation

Abstract: This narrative review explores the extent of the current knowledge of soft tissue barriers around implants from both a basic and clinical perspective, and aims to consolidate this knowledge and highlight the most pertinent questions relating to this area of research.

“…These suggest that we should not be overly optimistic when assessing esthetic situation, especially soft tissue management, while the discrepancy could be due to much harder to correct pink-tissue complications caused by various errors committed before, during, or after the placement of implants, rather than due to restorative failure. 39 The level of the soft tissue margin and soft tissue contour were the toughest variables to satisfy in the natural teeth, which was consistent with the findings of Fürhauser and colleagues. 22,35 Regarding the variables of PES/WES evaluations, the underlying strengths and weaknesses of the natural teeth were revealed in this study.…”

“…This was partly due to the shifting nature of the bone level after implant surgery, which was considered a foreign body reaction, contributing to weaker functional sealing capacity than natural teeth. 39 The level of the soft tissue margin and soft tissue contour were the toughest variables to satisfy in the natural teeth, which was consistent with the findings of Fürhauser and colleagues. 6 This was noteworthy that when selecting implant modalities, special care should be taken to preserve the soft tissue margin, which was a weak spot, even in natural teeth.…”

“…The method of HRP for histochemical staining by DAB method was following a previous study. 15 In this study, the vertical distance between the top of the PIE and the bottom of the HRP-positive area was determined in a direction parallel to the implant surface by third party.…”

“…In PICT, connective tissue fibrils are oriented parallel to the implant body and there is no binding to the implant surface (Atsuta et al, 2016;Berglundh et al, 1991). Supracrestal connective tissue fiber in PCT has a perpendicular orientation to the tooth surface and strongly adheres to the cementum by Sharpey's fibers, which leads to resistance against bacterial invasion.…”

In PICT, connective tissue fibrils are oriented parallel to the implant body and there is no binding to the implant surface (Atsuta et al, 2016;Berglundh et al, 1991). Furthermore, PICT contains collagen type V, which has greater resistance to collagenase than natural tissue, and PICT is generally considered to represent a chronic inflammatory condition (Romanos, Schröter-Kermani, Weingart, & Strub, 1995).…”
