Software updates of OCT segmentation algorithms influence longitudinal assessment of retinal atrophy

Abstract: The data show a significant difference in pRNFL thickness measurements between two subsequent versions of the same segmentation software. Although the mean difference was relatively small, the differences within the individual subject could be considerably higher than the known atrophy rate of 1μm/year in MS.

“…A sensitivity analysis excluding patients with a previous history of ON in either eye was also performed [11,12]. We studied the association of OCT measures, MRI volumetric measures and clinico-radiological variables by using Pearson correlation tests, adjusting for OCT software [18], MRI scanner and age when appropriate. We analyzed whether OCTderived measures and MRI volumetric measures were independently associated with disability by using multivariable logistic regression models with Expanded Disability Status Scale (EDSS) score as dependent variable with a threshold of ≥3.0 [19].…”

“…In any case, scanner type was included as a covariate in all statistical analyses that included MRI measures. Similarly, a recent work suggested that the use of different OCT segmentation software versions could affect pRNFL estimations [18]. Although this effect is probably not relevant in population studies, we decided to include the OCT segmentation version as a covariate in all statistical analyses with OCT measures.…”

“…A common limitation of multi-centre studies is the difference in methodology among the participating centres. 29,30 The OCT device and software were the same for all centres (Spectralis) but one (Topcon). The data on retinal layer thickness of this particular centre were not significantly different from the other centres', and additional adjustment for a potential centre effect did not change any of the results (data not shown).…”

“…The most important limitation is that we included OCT scans from only one device and one scan protocol, which limits generalizability of normative data (47). Particular caution should be taken when interpreting data acquired with various instruments, since comparative studies revealed that measurements are not directly comparable between different OCT devices (48, 49) and results can even be influenced by simple software upgrades (50). Currently, SAMIRIX is only able to work with the HEYEX Vol file format (*.vol files), which is only available through specific collaborative arrangements with Heidelberg Engineering, which is a clear limitation.…”
