Soluble IL-2 receptor levels in patients with Graves'ophthalmopathy

Abstract: SUMMARYIn various autoimmune diseases circulating levels of soluble 1L-2 receptor {si L-2R) seem to be related lo disease activity. Because reliable parameters of disease activity in Graves' ophthaltnop:ithy are lacking, we measured slL-2R levels in 47 patients witb this disorder, Tbe patients had Gruves" disease, but no otber iminune-mediated diseases, had not yet received specilic treatment for their ophthalmopathy and were euthyroid during the entire study period. Twenty-one ofthe 47 patients (45%) had sIL-… Show more

“…Changes in serum cytokines have been also observed in GO. In patients with elevated sIL‐2R a better response to corticosteroid therapy was observed [15], and IL‐6, CD30 and TNF‐α receptor I had some value for predicting a therapeutic outcome of orbital irradiation [16]. In our study no significant relationship was found for any measured parameter to the development and course of GO.…”

“…Although these levels failed to correlate with disease activity or severity, those individuals with the highest levels exhibited enlarged extraocular muscles. 87 Serum concentrations of IL-6 and soluble IL-6R were found to be elevated in GD compared to healthy individuals. 88 These were even higher in the subset with active TAO.…”

“…85 Messenger RNA encoding cytokines, including TNFa, IL-1b, IFNc, IL-4, IL-6, and IL-10, was detected in extraocular muscle and fat from patients with TAO. 86 Prummel et al 87 found elevated serum soluble IL-2R (sIL-2R) levels in approximately 45% of individuals with TAO. Although these levels failed to correlate with disease activity or severity, those individuals with the highest levels exhibited enlarged extraocular muscles.…”

“…Nonetheless, if there is a cross-reactive antigen in both thyroid and eye muscle, blood-borne T cells might be expected to respond to such an antigen, as thyroid antigen-specific T cells can be identified (albeit at low level) in the circulation, particularly after removal of the non-specific suppressor infiuence of the CD8'*' subset [17,20,21]. Furthermore, an increase in soluble IL-2 receptor levels and a decrease in CD8'^ T cell numbers can be detected in the circulation of patients with severe TAO [22,23], indicating that the ostensibly localized process in ophthalmopathy can produce detectable effects in the blood.…”
