Special ultrasound phantom for interventional training: Construction, advantages, and application

Training medical students in surgical procedures and evaluating their performance are both necessary steps to ensure the safety and efficacy of surgeries. Traditionally, trainees practiced on live patients, cadavers or animals under the supervision of skilled physicians, but realistic anatomical phantom models have provided a low-cost alternative because of the advance of material technology that mimics multi-layer tissue structures. This setup provides safer and more efficient training. Many research prototypes of phantom models allow rapid in-house prototyping for specific geometries and tissue properties. The gel-based method and 3D printing-based method are two major methods for developing phantom prototypes. This study excluded virtual reality based technologies and focused on physical phantoms, total 189 works published between 2015 and 2020 on anatomical phantom prototypes made for interventional radiology were reviewed in terms of their functions and applications. The phantom prototypes were first categorized based on fabrication methods and then subcategorized based on the organ or body part they simulated; the paper is organized accordingly. Engineering specifications and applications were analyzed and summarized for each study. Finally, current challenges in the development of phantom models and directions for future work were discussed.

Objetivo: Evaluar un simulador artesanal para entrenamiento de veterinarios en punción guiada mediante ultrasonografía en Quito, entre julio-septiembre de 2022. Metodología: Se elaboró 5 simuladores artesanales con diferentes ecotexturas como: útero grávido, vasos sanguíneos, tejido blando, inclusiones de cuerpos extraños y quistes, estos fueron colocados en recipientes de espuma blanca de 500ml, en una base de gelatina comercial sabor uva color oscuro, con sémola de maíz a fin de simular diferentes ecogenicidades y un antiséptico germidal®, fueron entregados en cinco centros especializados en ultrasonografía veterinaria de Quito, Pichincha, Ecuador. La metodología es cuantitativa, aplicando un cuestionario estandarizado con 10 preguntas de tipo cerradas, entregadas a veterinarios ecografistas de estos centros. El análisis estadístico se realizó utilizando medidas de tendencia central y analizando los porcentajes que determinaron las conclusiones. Mediante el programa SPSS®, elaborando tablas y gráficos 3D con los resultados; procediendo a interpretarlos generando las conclusiones. Resultados: el 60% de los encuestados está dispuesto a usar un simulador casero, el 20% lo usaría de forma probable y el 20% restante no lo usaría al no considerarlo atractivo. Un 40% preferiría usar este simulador versus uno comercial, lo cual a decir de Pereira da Silva et al. (2015), depende del centro de enseñanza y sus necesidades de entrenamiento, el 80% no dispone simuladores, en tanto que el 100% concuerda que el uso de un simulador mejora el aprendizaje de punciones eco-guiadas, considerando que el entrenamiento en grado no es suficiente a decir del 80% de los encuestados, de igual forma el 80% manifiesta que el simulador tiene una buena durabilidad de 90 días. Conclusiones: el 80% los médicos imageneólogos concluyeron que el simulador (fantoma) muestra propiedades acústicas similares a las de los animales permitiendo el entrenamiento en la toma de muestras guiadas por ultrasonografía.

BackgroundUltrasound is becoming more widely utilized in clinical practice; however, its effectiveness is limited by the operator's skills. Simulation models are attractive options for developing skills because they allow inexperienced users to practice without the risk of endangering patients. ObjectiveThe purpose of this study was to identify commercially available and homemade ultrasound models to describe them in terms of materials, cost, and whether they are high-or low-fidelity for medical student education. MethodsThis is an investigational study on cost-effective ultrasound training methods for medical students. Our study was performed using search engines in Google, Google Scholar, and PubMed to search for models for the following five modalities: foreign body identification, intravenous (IV) injection training, abdominal ultrasound, ocular ultrasound, and ultrasound-guided lumbar puncture training. ResultsMost homemade models for foreign body identification, IV injection training, and ocular ultrasound could be created for less than $20. IV injection training models were the cheapest commercially available models. There are multiple commercially available options for abdominal ultrasound models, but no options were found for homemade construction. The construction cost for lumbar puncture models was larger due to the need to purchase an anatomically accurate set of lumbar vertebrae. ConclusionsThis study provides initial guidance and suggestions for ultrasound training models that are currently available. Ultrasound models that can be cheaply made or purchased increase accessibility for medical students to gain early exposure in a cost-effective and safe manner.
