Specific HLA-DR4-associated histocompatibility molecules characterize patients with seropositive juvenile rheumatoid arthritis.

Abstract: A bstract. The structural and functional heterogeneity of HLA-DR4-associated specificities was investigated in patients with seropositive juvenile rheumatoid arthritis, a DR4-associated disease. Using a combination of HLA-D analysis by mixed lymphocyte culture and electrophoretic analysis of immunoprecipitated Ia molecules by two-dimensional polyacrylamide gels, we observed a surprisingly homogeneous pattern of HLA-D antigen expression. All patients expressed common structural products of the DR and DS loci, a… Show more

“…Thus, homozygosity ofthe DRB 1*0401 allelic variant might potentially be used as a prognostic marker in patients diagnosed with RA. A codominant function of HLA-DRBl alleles has been reported for children with seropositive juvenile arthritis who had a higher than expected frequency of carrying both the B 1*0401 and B1*0404 alleles (23). These observations suggest that such a double dose ofsusceptibility genes may predispose to an earlier onset of the disease and to more severe disease manifestations.…”

“…including antinuclear antibodies, rheumatoid factor, and antilymphocyte antibodies ( 1-91. Furthermore, disease susceptibility may be linked to both class 1 and class I1 major histocompatibility loci, in keeping with an inherited immunoregulatory defect (10)(11)(12)(13). These immunoregulatory abnormalities support the concept that therapy aimed at modulating the immune or inflammatory process is beneficial in the treatment of JRA.…”

“…This disease is similar to the adult form of seropositive RA. Class II molecules on the cells from the JRA patient had been previously characterized by detailed cellular typing and biochemical analysis (25). A panel of seven different T cell clones was developed.…”
