Spinal Anestezi Single Shot Spinal Anaesthesia for Caesarean Delivery of Two Achondroplasic Parturients

Abstract: Because of structural defects in achondroplasia, as one of the dwarfism types, the choice of an anaesthetic technique and management for caesarean delivery presents a challenge. We aimed to discuss our successful single-shot spinal anaesthesia approach for caesarean section for one urgent and one elective achondroplasic parturient based on the literature. Keywords: Achondroplasia, caesarean section, spinal anaesthesia Cücelik tiplerinden biri olan akondroplazide yapısal bozukluklar nedeniyle sezaryenle doğum iç… Show more

“…Many other factors were also considered, such as blood present in the cerebrospinal fluid, emergency cesarean section, multiple trials, bupivacaine dose, duration of surgery, prior anesthesia, spinal needle type and size, and bupivacaine baricity. This is due to the heavy pressure of unsuccessful spinal anesthesia [ [7] , [8] , [9] , [10] , [11] ].…”

“…However, several case reports of achondroplastic patients who underwent spinal anesthesia have been reported in the literature with reduced doses of local anesthetics (Table 1) [4][5][6][7][8][9][10]. Since the volume and dose needed for an adequate surgical block are difficult to determine as a result of the short stature of the patients, some clinicians advocate for a titratable technique such as epidural or combined spinal-epidural, rather than a single-shot spinal injection [9]. In our case, however, the patient presented to the operating room for a relatively brief operative procedure, and spinal injection was chosen over epidural catheter placement.…”

“…[ 1 ] Though encouraging literature is available on use of epidural, reports on spinal anaesthesia remain largely inconclusive. [ 2 3 ] Cord compression, disc prolapse with resulting disc herniation and paraplegia have been reported. [ 3 ] Free flow of CSF may also be difficult to obtain in these patients.…”
