Spinal metallosis: a systematic review

Abstract: Spinal metallosis can present weeks to years after spinal instrumentation surgery and is a potential cause of neural compression. This process appears to be independent of the instrument composition as metallosis has now been demonstrated in both titanium and stainless steel constructs.

“…It is well know that titanium particles and ions from medical implants can be dispersed widely in the body and there is also ample evidence that titanium particles are associated with localized inflammatory responses around medical implants . The frequently observed phenomena of inflammation associated with metallic particles around failing medical implant prosthesis has been termed metallosis . The localized inflammation, loss of bone attachment, and production of granulation tissue around a failing medical implant clinically resembles the conditions found around a failing dental implant.…”

“…For example, one study has demonstrated that bone fracture healing is impaired if there is a foreign metallic bullet [57]. Local metallosis is a well-known complication of metallic implants due to deposition of metallic parts and building up in the soft tissues [58]. Metallosis has been shown to affect nearby neural structures by causing granuloma/pseudotumor formation with mechanical compression [5964].…”

“…The imaging exam of choice is the CT myelogram. Artifacts may occur in CT scans and MRIs due to the presence of metallic material, which makes the assessment more difficult 5,6 . The CT highlights bone erosion, spinal instability, and spinal canal impairment.…”
