Spiral Computed Tomography for the Diagnosis of Cervical, Thoracic, and Lumbar Spine Fractures: Its Time has Come

Abstract: SCT of the spine identified 99.3% of all fractures of the cervical, thoracic, and lumbar spine, and those missed by SCT required minimal or no treatment. SCT is a sensitive diagnostic test for the identification of SF. Routine plain radiographs of the spine are not necessary in the evaluation of blunt trauma patients.

“…Brown et al [4] reported that a spiral CT scan was suitable to completely replace plain radiographs for thoracolumbar screening. Griffey et al [8] subsequently reported that CT scan thoracolumbar reformats had completely replaced plain films for screening at their Level 1 Trauma Center.…”

“…For example, as more individuals survive MVCs, lower extremity injuries have become more significant [7]. Conversely, more sensitive imaging, especially for the thoracic and lumbar spine, may account for increasing recognition of thoracolumbar injuries in MVC survivors [4,8].…”

“…To that end, therapeutic algorithms need to address the outermost importance of SI in MI-patients including a standardized diagnostic work-up of the spine. The exact diagnostic procedures to achieve this result remain a matter of discussion -CT-scanning is available in most traumacenters and meanwhile considered the golden standard [26] due to lower cost [27], diagnostic accuracy [22,[28][29][30][31] and speed [32], however, others still employ plain radiographs [33] especially for screening the axial skeleton before performing CT-scans of a suspicious region [34].…”

“…It is possible to show fractures missed by plain films. 9,10 On the other hand, MSCT has some disadvantages including high cost, requirement of considerable time for evaluation, and the risks of radiation. Moreover, there are some unclear points concerning the appropriate patient groups (lack of evidencebased information etc.)…”
