The incidence of colorectal cancer (CRC) is characterized by rapid declines in the wake of widespread screening. Colonoscopy is the gold standard for CRC screening, but its accuracy is related to high quality of bowel preparation (BP). In this review, we aimed to summarized the current strategy to increase bowel cleansing before colonoscopy. Newly bowel cleansing agents were developed with the same efficacy of previous agent but requiring less amount of liquid to improve patients’ acceptability. The role of the diet before colonoscopy was also changed, as well the contribution of educational intervention and the use of adjunctive drugs to improve patients’ tolerance and/or quality of BP. The review also described BP in special situations, as lower gastrointestinal bleeding, elderly people, patients with chronic kidney disease, patients with inflammatory bowel disease, patients with congestive heart failure, inpatient, patient with previous bowel resection, pregnant/lactating patients. The review underlined the quality of BP should be described using a validate scale in colonoscopy report and it explored the available scales. Finally, the review explored the possible contribution of bowel cleansing in post-colonoscopy syndrome that can be related by a transient alteration of gut microbiota. Moreover, the study underlined several points needed to further investigations.

Background:Simethicone can improve bowel preparation quality, but the optimal timing of oral simethicone before colonoscopy has not been determined. This study aimed to explore the effect of the time interval between oral simethicone and the start of colonoscopy (S-C) on bowel preparation quality. Material/Methods:A total of 364 patients undergoing colonoscopy at our department from August 1, 2021 to November 30, 2021 were included in the training cohort, and 420 consecutive patients from December 15, 2021 to January 31, 2022 comprised the validation cohort. They were classified into short and long S-C groups according to the median S-C. Bowel preparation quality evaluated by the Boston Bowel Preparation Scale was compared between the 2 groups. Logistic regression analyses were performed to explore the correlation between S-C and bowel preparation quality, and we explored the effect of run-way time and time of starting colonoscopy on bowel preparation quality. Results:In the training cohort, 182 and 182 patients were classified into the short and long S-C groups, respectively; in the validation cohort, 210 and 210 patients were classified into the 2 groups, respectively. In the 2 cohorts, the short S-C group had a significantly higher rate of adequate/excellent bowel preparation than the long S-C group. Logistic regression analyses showed that shorter S-C, shorter run-way time, and colonoscopy in the morning were all correlated with adequate/excellent bowel preparation. Conclusions:Bowel preparation quality may be affected by S-C, run-way time, and time of starting colonoscopy. S-C shortening should be given equal importance as run-way time shortening.

The Use of Enteral Nutrition in Bowel Cleansing Before Colonoscopy

ЦЕЛЬ ИССЛЕДОВАНИЯ. Оценить эффективность подготовки к колоноскопии пациентов, использовавших накануне исследования в качестве единственного источника энтерального питания Нутридринк (по критериям качества очистки толстой кишки и комфорта в употреблении препарата). МАТЕРИАЛЫ И МЕТОДЫ. В отделении эндоскопической диагностики и хирургии ФГБУ «ГНЦК им. А.Н.Рыжих» МЗ РФ был проведен открытый проспективный сравнительный анализ качества подготовки толстой кишки у 150 пациентов (67 мужчин и 83 женщины, в возрасте от 20 до 65 лет), которым была выполнена диагностическая колоноскопия после белковой диеты или энтерального питания Нутридринк. Для оценки степени очищения кишечника использовалась шкала Лайкерта. Субъективная оценка комфортности проводилась самим пациентом по 10-бальной шкале (от 0 -«отлично» до 10 -«крайне негативно»). РЕЗУЛЬТАТЫ. Качество подготовки толстой кишки к колоноскопии по шкале Лайкерта было достоверно лучше у пациентов, принимавших в качестве подготовительной диеты энтеральное питание Нутридринк, особенно по сравнению с пациентами, принимавшими стандартную белковую диету. Субъективная оценка комфортности также оказалась наилучшей в группе пациентов, использовавших Нутридринк как единственный источник питания. ЗАКЛЮЧЕНИЕ. На основании полученных результатов исследования можно сделать вывод, что применение энтерального питания Нутридринк оправдано в качестве замены белковой диеты при подготовке к колоноскопии.AIM: to evaluate the effectiveness of bowel cleansing before colonoscopy using enteral nutrition agent «Nutridrink» as the single nutrient. MATERIALS AND METHODS: a prospective comparative study included 150 patients (83 females, aged 20-65 years), who underwent diagnostic colonoscopy. To assess the quality of bowel cleansing the Likert scale was used. Subjective assessment of patients' comfort was performed by patients on a 10-point visual scale (from 0 -«excellent» to 10 -«extremely negative»). RESULTS: the quality of colon cleansing before colonoscopy was significantly better in patients who got enteral nutrition, especially in comparison with patients with standard protein diet. Subjective assessment of comfort during cleansing was better in the group of patients who used Nutridrink as the only source of nutrition as well. CONCLUSION: use of enteral nutrition as a part of bowel cleansing before colonoscopy can replace the protein diet.
