Spontaneous bilateral patellar tendon rupture: a case report and review of the literature

Abstract: Bilateral rupture of the patellar tendon is a very rare injury. It occurs in association with chronic systemic diseases or corticosteroid medications. We report a case of a 13-year-old child with Ehlers-Danlos syndrome presenting a bilateral patellar tendon disruption of proximal insertion that occurred with a trivial trauma. Surgical management consisting in tendon repair with a suture anchor technique protected temporarily with a cerclage wiring gives a good outcome.

“…Therefore, children should be at a high risk of strain-induced tendon injury. Paradoxically, however, the reported incidence of tendon injury is much lower in children than adults (Houshian et al, 1998;Moretti et al, 2008), which may reflect a degeneration with age or localised ''stress shielding'' in adult tendons .…”

“…For this reason, it is rare to find rupture of the tendon substance in children or adolescents. In the proximal region of the patellar ligament, the most common lesion is sleeve fracture, while in the distal region, we saw tibial tuberosity avulsion (12) . At our hospital, because it is a level I emergency hospital, we comorbidities, two with bilateral lesions, and four with associated lesions.…”
