Spontaneous hemodynamic improvement or stabilization and associated biopsy findings in patients with congestive cardiomyopathy.

Abstract: The hemodynamic courses of 56 patients with congestive cardiomyopathy (CCM) were investigated. Fourteen patients died within 24 months after diagnosis. The hemodynamic courses of the remaining 42 patients were investigated in subsequent examinations by determination of left ventricular ejection fraction (LVEF), mean pulmonary arterial pressure at maximal workload, and peak systolic pressure/end-systolic volume index. During the study interval of 32.2 + 20.0 months the conditions of 20 patients (48%) deteriorat… Show more

“…In non-renal failure patients there is a wide variation in the natural history, which is inherent in the definition of the disorder as it has many causes [21], The majority of patients have a poor prognosis but some (13-39%) make a remarkable recovery or stabilize [22][23][24][25]. In our study 24% had improvement in cardiac function.…”

“…5 The prognosis in classical dilated congestive cardiomyopathy has been said to be negatively related to ventricular size.9-12 Prognosis also has been said to be better in patients with preserved myofibrils in endomyocardial biopsy. 13 We have previously described an unusual, probably variant, form of dilated congestive cardiomyopathy termed "mildly dilated congestive cardiomyopathy" (MCDM), for lack of a better term. This was initially described in five heart transplant recipients with persistent clinical and hemodynamic features of end-stage congestive cardiomyopathy but with little or no left ventricular dilation.14 Additional unusual findings of MDCM were little or no myofibrillar loss on electron microscopy and a trend toward occurrence of dilated cardiomyopathy in other family members.…”

“…Patients with a history of hypertension, systemic disorders or a known infection were excluded from the study. Left ventricular and coronary angiography and Swan-Ganz catheterization were performed according to standard methods [Figulla et al, 1985]. …”
