Spontaneous septicaemia with multi—organ dysfunction — a new face for Pantoe agglomerans?

Abstract: Pantoe agglomerans (P. agglomerans) is an unusual cause for sepsis in immunocompetent individuals, especially in the absence of characteristic risk factors. We report one such case occurring in a farmer, manifesting with severe illness. The severe nature of illness and the apparently spontaneous origin of septicemia underline the pathogenic potential of this organism. When coupled with the ubiquity of the organism, there is a definite possibility that this disease may become increasingly frequent in the near f… Show more

“…Sporadic adult and pediatric cases were seen in equal proportions, with immunocompetent patients greatly outnumbering their immunocompromised counterparts. Bacteraemia, 14–36 followed by skin and soft tissue infections (SSTI), 31,32,37–42 were the commonest manifestations of Pantoea infection, but the range of syndromes was wide, including peritonitis, 32,43–52 bone and joint infection, 32,53–60 intra‐abdominal abscess, 61–63 pneumonia, 31,64 urinary tract infection, 31,32 ocular infection, 65–71 and rhinosinusitis 72 . One case each of endocarditis, 15 prosthetic joint infection, 55 and post‐neurosurgical meningitis 73 was also reported, as were four cases of neonatal sepsis 74,75 .…”

“…In contrast to the proven pathologic role, some beneficial traits of P. agglomerans have been mentioned recently for their role in antibiotic production as an immune potentiator in cancer patients, as a biopesticide and food preservation [15]. e clinical course of the disease in most adult cases is usually mild, unlike the pediatric population; however, it can cause severe illness leading to septicemia, DIC, and multi-organ failure [16]. Application of the proper antibiotic treatment leads to full recovery, even in immunocompromised individuals, as we saw in our patient [2].…”
